not be accepted withotrt a copy of�he DRB approval form attached (if applicable�. :
e If this permit is for installation of an exterdor hot tub or spa on a new elevated p4atform or deck over 30"
abvve grade, you must also obtain a builciing permit.
e If this permit is for installation of an exterior hot tub or spa on any existing deck or elevated piatFarm, a.
structural erlgineer must r�view the existing conditior� and verify fhat it will Supp�rt the addec�
concentrated load, P6ease provide a copy of the structural enginaers wet stamped letter c�t drawing
with this application. ,
e If this is a remodel in a multi-family building with a hameowners associativn, a letter of permission firom
the association is required.
