General Information: This application is required for all proposals involving minor changes to buildings and site improve-
ments, such as roofing, painting,window additions, landscaping,fences, retaining walls,etc. Applicable Vail Town
Code sections can be fodnd at vrww.vai, Igov.com under Vail Information–Town Code Online. All projects requiring de-
sign review must receive approval prior to submitting a building permit application. An application for Design Review
cannot be accepted until all required information is received by the Community Development Department, as outlined in
the submittal requirements. The project may also need to be reviewed by the Town Council and/or the Planning and
Environmental Commission. Design review approval expires one year from the date of approval, unless a building per-
mit is issued and construction commences.

RE:Vail Golf Course#18 Golf hole renovation
The Vail Recreation District(VRD), in conjunction with the Town of Vail(TOV)is currently working on plans to
realign the#18 golf hole on the Vail Golf Course.
Background:
The existing#18 golf hole currently presents several challenges that need to be addressed. These include errant
golf balls being played on the existing 18 hitting the golf club house as well as general public playing the 18`h hole
being hit by golf balls coming over the driving range nets.
TOV contracted Phelps/Atkinson golf design group to provide the final design for a new#18 golf hole. Included in
the design is anew retaining wall to provide additional elevation for the new black tee. We will also be removing
a few trees however the plan calls for a significant number of new trees to be installed to enhance the golf hole.
This project will result in more trees on the golf course.
Purpose:
At the regular Vail Town Council meeting of July 17,2012,the Vail Town Council voted 7 to 0 to move the 18`h hole
to address existing safety concerns regarding the current alignment of the 18`h hole.
Details:
This project will involve excavating areas of the existing#18 to install new golf feature including bunkers,tees and
greens. This project also involves the removal of several trees on the proposed black tee and fairway.The plan
also involves the installation of 32 new trees as described in the tree plan. We will also be installing a new
retaining wall to elevate the black tee. The retaining wall will not exceed 4'and will be constructed with the
Rosetta Stone material which was previously approved for use on the Vail golf course.

Note: Pursuant to CRS 10-11-122, notice is hereby given that:

Note: Effective September 1, 1997, CRS 30-10-406 requires that all documents received for recording or filing

Company conducts the closing of the insured transaction and is responsible for recording the

E) If there has been construction, improvements or mayor repairs undertaken on the property to be purchased
within six months prior to the Date of the Commitment, the requirements to obtain coverage
for unrecorded liens will include: disclosure of certain construction information; financial information
as to the seller, the builder and or the contractor; payment of the appropriate premium fully
executed Indemnity Agreements satisfactory to the company, and, any additional requirements
as may be necessary after an examination of the aforesaid information by the Company.
No coverage will be given under any circumstances for labor or material for which the insured
has contracted for or agreed to pay.
Note: Pursuant to CRS 10-11-123, notice is hereby given:
This notice applies to owner's policy commitments containing a mineral severance instrument
exception, or exceptions, in Schedule B, Section 2.
A) That there is recorded evidence that a mineral estate has been severed, leased, or otherwise
conveyed from the surface estate and that there is a substantial likelihood that a third party
holds some or all interest in oil, gas, other minerals, or geothermal energy in the property; and
B) That such mineral estate may include the right to enter and use the property without the
surface owner's permission.
