o All installations of ext��-ior hot tubs or spa`s re�uire a DRB app�roval from planning.This application will
na� be accepted witho�t a copy af the DRB approval form attac�ed (if applicable},
❑ If th�s permit is for installatian af an exterior hot tub or spa on a n�w elevated plat�orm ar deck over
30" above grade, yo� must also obtain a building permit.
o If tl�is permit is for installation of an exterior hot tub ar spa on ar�y existing deck ar elevated platForm,
a structural engineer must review the existing cvndition a�d verify �hat it w�ll support �he added
concentrated load. PSease provide a copy of the s�ructural engineers wet stamped letter or drawing
with this application.
o If this is a remod�l in a mu�ti-farnily building with a homeawners association, a letter af permission
from the associ�tion is required.
o If this permit is for a commercial s�ace, two (2) sets of stamped drawings are required. E/ectrica/
one-/ine and�ane!schedu/es are required ifload is added or dist�ib�►fion is�/tered,
