Drawer 5400 Avon, Colorado 81620 (cco ll ins@ganelso n.corn) Job No. 112 316A Subject: Summary of Observation and Construction Testing Results, Ford Park Improvements, 530 South Frontage Road East, Vail, Colorado Dear Chuck: As requested, representatives of Hepworth-Pawlak Geotechnical observed the foundation excavations and performed construction materials testing at the subject site starting September 11, 2012 through June 13, 2013. We tested concrete placed at the site for retaining walls, building foundations floor slabs and pavement areas. We observed masonry and concrete cast-in-place concrete wall reinforcement. We tested masonry grout and prisms and observed masonry wall construction. We tested trench and wall backfill, and slab and pavement areas subgradc conditions and compaction. We also tested asphalt pavement materials and compaction. Additionally, we observed the construction and commented on the suitability of the MSE wall. At this time the project is essentially complete. The findings of our observations and testing results indicate that the construction is in substantial conformance with the plans and specifications. Recent concrete testing for exterior sidewalk areas is not complete at this time, but based on our experience with the project mix we do not anticipate low test results with the sidewalk concrete.
