General Information: This approval is granted for the removal of dead or diseased trees only. A separate
application is required to request tree removal/replacement in the Town of Vail. This form must be signed by a
Town of Vail authorized representative who has inspected the tree(s). To request an inspection, please call
Tom Talbot, Wildland Coordinator at (970) 477-3509.
Applicant has 30 days from the date of this application to submit a mitigation plan to the Town of Vail Wildland ��

General Information: This application is to request tree removal in the Town of�/ail. As part of this application,
the property owner may be required to replace trees that are removed. If required to replace, applicants must
replant trees by November 1 st of the following year from the date of approval. Please be prepared to provide a
tree replacement plan. Please see tips for tree planting and species seleciion on next pa^ye. Des�gn review
approval expires one year from date of approval.

ownership properties such as duplex, condominium, and multi-tenant buildings. This form, or similar
written correspondence, must be completed by the adjoining duplex unit owner or the authorized agent of
the home owner's association in the case o1 a condominium or multi-tenant building. All completed forms
must be submitted with the applicants completed application.
