If this permit requires a Town of V�il Fire Department Approval, Engineer's (Public Worksj
review and approval, a Pla�ning D�parkment review or Health Department review, and a review
by the Building Department, the estimated time for a tatal rewiew will take as fong as three (3)
weeks.
A{I commercial (darge or small) and all multi-famiiy pe�mits will have to follow the above
mentioned maximum �equirements. Residential and small projects should take a lesser amount

PUBLIC 1N�RKS AND THE PUBLIC WAY PERMIT PRO�CESS
How it relates to Suilding I'ernrtits:
� Fill out the attached check list with the Buildinq Permit Applicat�on.
If yes was answered to any of the questions then a "Public Way" permit is required. You
can pick up an applicatian at eit�er Community Development, located at 75 South Frontage
Road or Public Warks, locatec! at 1309 Elkhorn Drive.
� Notice sign-offs for utility companies. ALL utilities must feld verify (locate) respective
utilities prior to signing appl�cation. Some utifity companies require up to 48 hours notice to
schedule a locate.
� A construction traffic control/staging �lan must be prepared on a separate sheet of paper.
An approved site plan may also be used. This plan will show lacations of all traffic control
devices (s�gns, cones, etc.} and the work zone, (area of construction, staging, etc.). This
plan will expire on Nov�mber 1st and will need to b� resubmitted for consideration for
appraval through the vwinter. Be aware that your resubmission for wi�ter may be denied
depending an the location of construction.
i= Sketch of work being �erformed must be submitted indieating dimensions (length, widtl�
and depth of work). This may be drawn on the traffic control plan or a site plan for t�e job.
: Submit eompleted application ta the P'ublic Wor�C's office for review. If required, locates will
be scheduled for the Town of Vail electricians and irrigation crew. The locates take plac� in
the morning, but may require up to 48 hours to perform.
� The Public Work's Constructio� Inspector will rev3ew the applicatian anc� approve or deny
the permit. You wil{ be contacted as to the status ar�d any requirements that may be
needed. Most permits are released within 48 t�ours of being received, but please allow u�
to one (1) week to process.
;� As soon as the pe�mit is approved, t�te Building Department will be notifi�d, allowing the

Construction Hours regulations at anytime should abuses or problems arise.

Attached is the report for the asbestos survey and bulk sampling of suspected asbestas-
containing building materials at The Lodge At Vail, #404, 174 Gore Creek Drive, Vail,
Colorado.
This investigation was conducted on behalf of and for the exclusive use of Mr. Joe
Rzetiejewslci & Co. (client), solely far use in a limited asbestos survey of the property.
This report and the findings herein shall not, in whale or in part, be disseminated or
conveyed to any other party, nor used by any other party, in wh�le or in part, without
priar written consent af Baulder Environmental tvianagement.
The results of this survey are timited ta materials that were assessable and foreseeabl�e.
Additional materials may e�ist behind hard surfaces. Personnel knowledgeable of
asbestos containing materials shduld always oversee demolition activities. Demolition
work should be halted and sampling performed by a state of Colorado licensed building
inspector if additiana� materiats, which are suspected of containing asbestos, are
encount�red.

BULK SAMPLIN� REPORT

Introduction:
On April 18, 2403, Boulder Environmez�tal Management, Inc.'s building inspector,
Chris Maron, conducted bulk sam lin of sus ected asbestos-containin materials under
the direction of our client. Sample locations were limited to areas accessible to the
inspector. The purpose af the sur�ey was to identify asbestos containing materials
(�4CM) and materials presumed to contain asbestos (PACM) as d�fined by the
Environmental Protection Agency(EPA.).
Sampling:
During the sampling pracess, suspect ACM was separated into three EPA categories.
These catego�ies are thermal system insulation (TSI), surfacing ACM, and miscellaneous
ACM. TSI includes all materials used to prevent h�at loss or gain or water condensation
an mechanical systerns. Examples of TSI are�ipe coverings, boiler insulation, duct
wrap, and mud packed fitting cement. Surfacing ACM includes all ACM that is sprayed,
trowelled, or otherwise applied to a surface. These applications are most cammonly us�d
in fireproofing, decorative and acoustical applications. Miseellaneous materials ine�ude
all ACM not listed in the thermal system insulation or surfacing categories, such as
linoleum, vinyl asbestos flooring, and ceiling tile.
Boulder Environmental Management, Inc. followed EPA recommended sampling
guidelines for identifieation of asbestfls in bulk matrices. A set of three (3), five (5), or
seven (7) samples were collected far each material type and analyzed by Polarized Light
h+licroscopy (PLM). Eac� sample set was systematically analyze�. Upon determination
that a sample in a set contained asbestos, analysis Qf the remaining samples in the set was
discontinued. If no asbestos was detected during the analyses, the suspect material was
determined to be negative for asbestos content.

Discussion:
Sample analyses resu�ts are reported in percex�tages of asbestos and non-asbestos
components. The EPA defines any material that contains greater than one pereent (1°10)
asbestos, utilizing PLM, as being an asbestos containing material. I'vlaterials tl�at are
identified as "nan detected" are specified as not containing asbestos.

Conc�usion:
The #7oor tile and mastic is not asbestos containing. The textured drywall and joint
compound contains a traee of asbestos. The demalition of trace asbestos materials
requires OSHA class 2 asbestos removal work practices. It is recor�mended that a
licensed asbestos abatement contractor be retained for this demolition to comply with
QSHA requirements regarding the demolition of asbestos containin� �naterials that
contain less tha�n 1% asbestos.
The OSHA regulation 29 CFI� 1926.1101(g)(1)(ii) and (iii), as well as record-keeping
requirements under 29 CFR 1926.1141(n) that are assQCiated with the negative exposure
assessment, apply so long as neither asbestos percnissibie exposure limit (PEL} is
exceeded or might be exceeded.
29 CF�Z 1926.1 l01(g)(1}(ii) requires:
"Wet methads, or wetting agents, to coatrol employee exposures during asbestos
handling, mixing, removal, cutting, application, and cleanup, except where
employers demonstrate that the use of wet methods is infeasible due ta for example,
the creation of electrical hazards, eq�ipment malfunetion, and, in roofing, except as
provide in paragraph (g)(8)(ii) of this section;"
and 29 CFR 1926.1101(g)(1){iii} requires:
"Prompt clean-up and disposal of wastes and debris contaminated with asbestos in
leak-tight containers except in roofi�ng o�rerations, where the procectures specified ir�
paragraph (g)(S}(ii) ot'this section apply."
