General Information: This application is to request tree removal in the Town of Vail. As part of this application,
the properly owner may be required to replace trees that are removed. If required to replace, applicants must
replant trees by November 1 st of the following year from the date of approval. Please be prepared to provide a
tree replacement plan. Please see tips for tree planting and species selection on next page. Design review
approval expires one year from date of approval.

ownership properties such as duplex, condominium, and multi- tenant buildings. This form, or similar
written correspondence, must be completed by the adjoining duplex unit owner or the authorized agent of
