Gore Creek Dr. Vail, CO 81657 Re: Food Service Plan Review Lancelot Bar Relocation Vail, Colorado 81657 Dear Mr. Schadl: This letter will serve to confirm the plan review of the above captioned food service establishment. The plans have been approved with the following additions and/or modifications: 1. The bar glass washer waste water shall discharge atmospherically by means of an air gap over a waste sink. If the glass washer has a waste water discharge air gap within the unit, a floor sink discharge is not necessary. 2. The cocktail unit cold plate must be built into the bottom of the sink or an integral part of the unit. 3. Condensate drain lines from all refrigeration units and ice machine(if installed)must have an air gap of at least two(2)pipe diameters at the drain. 4. Hand washing sinks shall be conveniently located to employees involved in bar and utensil handling. Sinks used for food preparation or for washing equipment shall not be used for hand washing. S. The minimum acceptable floor finish in the bar area is smooth, easily cleanable, light in color non-toxic gloss enamel or an epoxy-based paint. Quarry tile or epoxy base flooring is acceptable. 6. The minimum acceptable ceiling finish over the bar area is gloss enainel or epoxy-based paint on dry wall; or non perforated vinyl-coated dry wall, lay-in ceiling tiles. 7. The junctures between walls and floors shall be coved and sealed. S. Under counter surfaces must be finished and sealed to provide a washable surface. 9. All light fixtures must be adequately enclosed or shielded to protect foods in the storage, serving, or preparation areas from accidental bu16 or tube breakage. 10. Properly seal sinks and drain boards to their adjacent wall. Exposed screw heads and bolts are not approved. 11. All plumbing,electrical and other utility lines must be concealed in the walls, floor or ceiling. Water service lines extended to sinks may be surface mounted provided they are properly mounted on approved spacers at least 'h inch from the walls and 6 inches above the floor. 12. The dishwashers must be capable of a minimum temperature of 180 degrees F in the fmal rinse cycle or proper chemical final rinse cycle chemical concentration. 13. All equipment must be National Sanitation Foundation(NSF) listed or equivalent. Any changes to the plans as submitted and hereby approved must receive advance approval from this Department. A final inspection is required to determine is the kitchen is in compliance with the Colorado Retail Food Rules and Regulations. This report is specific to the events conducted on May 13, 2011, located at 201 E. Gore Creek Drive, Vai1, Colorado. Clearance services were conducted on all three floars of the house. The CBE on-site representative was respc�nsibie far consulting with the United representative, to ensure the a�atement areas and materials removed were understood so that clearances coulcf be properly perfarmed. CBE was not present during asbe�tQS remaval ac#ivities. Final V�sual Clearance Upon completian of asbestos removaf activities, a final visua( inspectian of the work area is requi�ed by the applicable regulations. A certifie� AMS must visually inspect the work afea for any dust or debris remaining in the area. If the inspector nates any such dust ar debris, the a�atement cantractor is required ta re-clean the work area until such time that the inspecting �MS does nat observe any dust ar debris in the work area. Mr. Mike Lc�coco cc�nducted the final visual inspection o# the site. Mr. Lococ� is certified {#13438) as an Asbestos lnspector, Abatement Designer, and Ai� Monitaring Special�st as specifiecf by the Asbestos Hazard Emergency Respanse Act (AHERA) and th� Colorado Department of Pub�ic Heal#h anci Envirt�nment (CDPHE}. It was determined that the abatement contractc�r had properfy cleaned the work area comple#e!y of all dust and debris. The inspecting AMS required no further cleaning. Fina! Clearance Air Monitaring and Sampling Methadolagy Final cleacance air samples must be caliected fram the work area after completior� raf a finai visual inspection and prior tc� teardown af the containmen# structure. Ana�lysis of each these sarnples must show airborne asbestos fiber concentrations of less than or equal to 0.010 flcc. lf concentrations exceed this limit, the abatement cantractor must re-clean the wark area and a second set af final clearanc� air samples must be coliected and analyzed. This process is repeated until the analytical resu�ts of the �nal ciearance air sampies meet the regulatory standard. �nly upon successfully compietion of the final clearance air mc�nitoring process may an asbestos abatement prc�;ect be cansideeed comp�ete, and teardawn af the contairtment structure can cornmence. Mr. Locaco compEeted the final clearance air sampling. Fi�a�l clearance air sampies were collected using cons#ant flow high-volume air sampling pumps, 25 mm air sampling cassettes, flexible tubing and adjustable sampling stands. The sampling cassettes were attached to one end c�f the ffexible tubing and placeti on the stands approximately forty-eight {48) inch�s above the #loor. The opposite ends of the flexib{e tubing were attached to the air sarr�pling purnps, Each pump was activated and �perated at a flow rate af approximately sixteen liters per minute (16 LPM� for a period of approximateiy eighty- five {85j minu#es. Aggressive air sampl�ng techniques, as described in 44 C�R Part 763, Appendix A tr� Subpart E, were utilized throughout the sample callection prQCess. Upon completion of the fina! ciearance air sampling, labels containing sample identi�cation numbers, sampie location, pump flaw rate, and total sample fime were applied to each sampling cassette. Per the applicable regulations, �ased on the quantity of ACM removed during the abatement project, five (�) finai clearance air samp{es were cal�ected in the full cantainment work area lacated at the site. Analytica! Methodofogy and Laboratory results The CBE representative is trained in P�►ase Contrast Micrascopy by Nationai Institute of Qccupationa} Safety {NlOSH) 582 Equivalent Course and successfultly participates in the American Industrial Hygiene Association {AIHA} Praficiency Anakytical Testing Program �PAT} for determining concentrations of airbome asbestos fibers (Lab !D 20�293a. The res�lts of the labaratary anaiysis of the final clearance air monitaring samples confirm tha# the concentrations Qf airbc�rne fibers within the work area were below the regulatory limit c�f Q_01 Q f/cc. Results below the detection limit are listed as BD�. A copy of the analyticai results of the fina! clearance air sarnpling is included in Append�x A of this Report. Conclusions: CBE pravided asbestc�s abatement clearance services in the building l�cated at �81 E. Gore Creek Drive, Vail, Calorada on May 13, 2011, Air samples were collected in a full containment until al! samples were below the regulatory level of 0.410 flcc and before teardown of cantainment was approved. Therefore, the work area was clean upon completian of the abatement project and is ready for re-occupancy. Limita#ions: This report describes the data collected and observations made by CBE during the final cleara�ce sampling �n the den of the above referenced building. CBE represents that its services were performed within the limits prescribed by applicable regulatians, within scope of work as approved by the EPA and the State of Calorado and in a manner consis#ent with the i�vel of care �nd skill ordinarily exercised by c�ther profiessional consultants under similar circumstances. The inspection was made, and the samp[es were collected by John R. Peterman, an A.H.E.R.A. and State of Colorado Certified Asbestos lnspector. Great care was taken during the inspection and samplin�to be as accurate as possible. lt should be noted that minimal dama�c was done to the existin� building struc:tures during the inspection so there is no documentation for unseen conditions or stored items. A1! samples were analyied by DCM Science Lab in Wheatridge, CO. This laboratory is deemed "Proficient" in the E.P.A.Quality Assurance (QA) program for the dctcrmination of asbestos in bulk materials, and is accredited by the American i lygiene Association {AHA). SAMPLING PROTOCOL: A random sampling scheme was used to sampie the suspect materials that were discovered.
