Dear Mike:
As requested, we have reviewed the structural steel reports made to date for the subject
project. The services are supplemental to our agreement for professional services to
Slopeside Construction, Inc. dated march 3, 2014.
The inspection of the structural steel connections for the project was performed by West
Slope Inspection and Testing (WSTI) as a subcontractor to Hepworth- Pawlak
Geotecluiical. Doug Young with WSTI, who performed the inspections, is a certified
inspector for welded and bolted connections. The structural steel construction is
essentially complete at this time.
One inspection report was made for a site visit to the project by WSTI on July 21, 2014.
A copy of the report is attached. The inspections indicate the structural steel connections
were made in general conformance with the project plans and specifications.

Inspection of shop fabrication or field work at job site: Field
Report: Visually inspected field welding at steel beam, column, stiffener, and
tube steel frame connections on the project. Note welding was inspected as best as
possible with wood framing & sheathing installed. Also there did not appear to be
any A325 high strength bolting to inspect on the project or any complete joint
penetration welds that needed ultrasonic testing.
The field welding inspected above appeared to be in substantial conformance
with the plans, and the quality requirements of the AWS D1.1 Structural Welding
Code.
To the best of my knowledge this completes the special inspections of the
structural steel onsite welding, as outlined above for the project.

Subject: Comments Regarding Helical Pier Capacity, Proposed Residence, Lot 49,

Dear Mike
As requested, we are providing our comments regarding load capacity of the installed
helical pier at the subject site. Hepworth- Pawlak Geotechnical previously observed the
foundation excavation for the residence currently under construction at the site and
submitted our findings in a report dated March 6, 2014 under the above job number. The
services are supplemental to our agreement for professional engineering services to
Slopeside Construction, Inc., dated March 3, 2014.
A single helical pier was installed on the east side of the residence to support a deck.
Design plan Sheet S2 by KRM Consultants dated February 2, 2014 (revised June 12,
2014), indicates the pier to have a needed 8 kip downward working capacity and an
ultimate downward capacity of 16 kips. The pier was installed on June 5, 2014 by Great
Divide Construction with hand equipment. Information provided by Great Divide
indicates the pier was a 1.5" square bar with a single helix and installed to a depth of
about 8 feet where torque refusal (8 pins sheared) occurred. The pier was installed in an
area of natural soils, that based on our excavation observation findings, consist of sand
and gravel cobbles.
Based on the information provided and our experience in the area, it is our opinion the
pier has adequate design and load capacity. There could be some differential settlement
with respect to the residence which should be considered in the design.

Dear Tom:
At the request from Michael Pukas, the architect, Justin J. Yarnell, PE, and Mark Luna,
from Martin/Martin, Inc.,we were asked to review the Grading Plan for compliance with the
grading recommendations outlined in the Soils and Foundation Investigation Report by LKP
Engineering, Inc., dated July 1, 2013, under the same project number. The scope of our services
included review of the drainage pattern established on the site, drainage swales, and spot
elevations. This review does not include, a drainage study or analysis of the existing or proposed
conditions on the site, and does not include review of the watershed and the capacities of the
proposed structures.
Our comments were marked on the pdf drawing they submitted, and e-mailed back to the
above-named team on August 16, 2013. After our meeting with Michael and Justin, Justin
revised the drawing and e-mailed it back to us on August 19, 2013. Following our final review
we have concluded that the grading and drainage shown on the site plan are satisfactory. It
provides for a positive runoff away from the building in all directions.
This report has been prepared according to locally accepted Professional Geotechnical
Engineering standards at this time. There is no other warranty either expressed or implied.
