SPECIAL 1NSPECTION AND 7ESTING AGREEMENT

The owner or his/her reptesentative, on the advice of the design professional in responsible charge, shall
comp(ete, seal, sign and submit a copy of the Special Inspectson Agr�ment and Structural Tests
Scheduied ta the Town af Vail for review and approval. Signatures are required on both pages;
photocopied or fexed signatures are accep#able.
The owner and his/her general contractc�r, whe�e appficable, shali also acknawledge the following
conditians applicable to Specia� Inspection Testing:
1. Cantractor is respo�sible for proper notifica#ian to the lnspection ar Testing age�cy for items
listed.(Page 1) {IBC 1704)
2. Only ihe testing laboratory shauld take samples and transport#hem to their laboratory.
3. Copies of all laboratary repo�ts and inspections are to be sent directly to the Tow� a#Vail by the T�sting
agency on a weekly basis.
4. Inspection agency to submit names and qualifications of on-site special inspectors to the Town of Vail
for review and approvaL( Page 2)
5. The special inspector is respansible to immediately notify the Tawn of Vail Building Official in writing of
any cancerns andlo�problems encountered.
6. 1t is the responsibil'tty of the contractor to review the Town of Vail approved plans for additional
inspection or testing requirements that may be noted. A pre-construction eonference at the job site is
recommended to review special inspection procedures.
7. The special inspector shall use only the Town of Vail approved drawings.
8. All special inspection field reports must be left on site for review by the Tgwn of Vai! s#aff prior to
required inspections or re-inspections.
BEFORE OCGUPANCY W1lL BE GRANTED: The special inspection agency shail submit a signed and
sealed statement that all items requiring testing and inspection were fulfrlled and reported. Those items nat
tested andlor inspected shal!be noted in this statement. A copy of#he s#atement shall be maintained at the
job site for the Building Inspector's review prior to final insReciian.

As you know, we met at the Sandstone Playground on Friday, December 16, 2011 to review the
welding on some of the erected equipment
The members that we reviewed on site have been assembled with less total weld than originally
specified on the structural construction documents. The attached Sheets S5, S6 and S7 have had
notes added depicting the locations of these deviations from the original documents. �

After reviewing these welds on site, I have determined that the existing amount of weld is adequate in ;_
all of these locations,and that no additional welding will be necessary. `;
