(GROUND) performed during the improvements to the Rojas residence (Snow Forest Chalet) at 186 Forest Road located in Vail, Colorado. The special inspection and materials testing services were performed at times scheduled by the Owner, Contractor and/or Subcontractors on February 14th, 2012 and according to GROUND's proposal # 1202- 0228 dated February 10th, 2012. During construction, a certified welding inspector (CWI) visited the project site on a part-time basis, when scheduled by the Contractor and/or Owner, to observe the structural steel field welded and fastened connections. We did not, at any time, observe any drilled and epoxy connections. Based on the field observations to date, RFI's received and/or project documentation received from the design team, it is our opinion that the materials observed for the structural steel were in general accordance with project plans, specifications, and/or applicable RFI's. The observations and materials testing as indicated above were performed periodically to provide data to the Owner, Contractor and Subcontractors to assist them in evaluating the general compliance of the materials and/or work with the contract documents at the times of our observation and testing. We did not supervise, direct or have any control over any of the Contractor's or Subcontractor's work. We were not responsible for the Contractor's or Subcontractor's scheduling of our services, means, methods, procedures, techniques, or sequences of construction, nor responsible for ensuring that any Contractor or Subcontractor performed or completed the work in accordance with the contract documents or construction industry standards. Box 2670 Vail, CO 81658 Attn: Mr. Mike Charles As requested, a representative from GROUND arrived at the project site to observe welded connections as directed by Ryan with Maximum Comfort Pool and Spa. The following is a summary of the observations performed on this date: 1) Observed anchor bolt connections of beam to existing wall and HSS tube to existing wall and found them to be in general compliance with the project plans. 2) Observed welded connections of beam to HSS 4"x8"x3/8" and HSS 4"x4"x3/8" to 4"x8"x3/8" and 4"x4"x3.8" cross member was not welded all around per Detail 3/S1. It is welded on three sides only. Discrepancy Item #1. All other areas were found to be in general compliance with AWS D1.1-2010 Clause 6 and project plans. 3) Detail 3/S1 shows HSS 7"x4"x1/4", HSS 8"x4"x3/8" was used. Discrepancy Item #2. 4) GROUND is requesting Welding Procedure Specification (WPS) for the welders on this project. Discrepancy Item #3. 5) GROUND received the Welder Qualification Test Record (WQTR). These items include: 1 . The HSS 4x4 header is only welded on three sides (rather than four sides per plan). 2. An HSS 8x4x3/8 was used for the cantilevered beams rather than HSS 7x4x1/4 per plan.
