❑ Underground 5ervices have to be in conduit (PVC) from the transformer to the electric meter, main disconr�ect
switch and to the first electricaf distribution circuit breaker panel.
❑ The main disconnect switch shall be located next to the meter an t�he exterior wall of the structure easily
accessible.

not be accepted without a copy af the DRB approval farrn attached (if applicable).

30" above grade, yau must also obtain a buiid�ng perm9t.
❑ If this permit is for ir�stallation of an exterior hot tub ar spa on any existing deck or elevated platform,
a structur�i engineer rnust review the existing conditi�n and verify t�ta� it will support the added
concentrated load. Please provide a copy of the structural engi�eers wet stamped letter or drawing
with this application.
❑ If this i� a remodel iro a multi-farnily b�ilding with a homeflwners �ssoc�ation, a ietter of permiss�on
from the association is required.
❑ If this permit is for a commercial space, two (2) sets of stamped drawings are required.
