Vail CO 126 Riverside Dr. Silverthorne, CO 80498 • SummitECinc@yahoo.com • 970.389.3362 SCOPE OF WORK: On April 2,2014 Dana Wallace with Summit Environmental Consulting Inc (SEC)was hired by Custom Refinements to conduct a limited asbestos inspection and collect bulk samples of suspect asbestos containing materials (ACM)within a portion of the multi-level single family residence located at 126 Forest Rd. in Vail CO.The purpose of the inspection was to determine if there was any asbestos in the drywall and surfacing materials prior to removal for remodel. COMPANY INFORMATION:SEC is a Colorado Department of Public Health and Environment(CDPHE) certified Asbestos Consulting firm, registration number ACF-19482 and Dana Wallace is a CDPHE certified building inspector having certification number 19272. Inspection procedures include identifying and sampling any suspect ACM within the area requested by the client,submitting such samples to a laboratory accredited by the National Voluntary Accreditation Program (NVLAP)for analysis, classifying the materials and assessing their condition, and writing a report detailing the inspection and analytical results of the bulk samples. Samples are taken in a statistically random manner of each homogeneous area, according to guidelines of the Environmental Protection Act(EPA)and CDPHE Regulation 8. Reg.8 III.A.d states "The Division recommends that all buildings be inspected prior to any renovation or demolition activities, regardless of the date of construction."A homogeneous area means uniform in appearance, color,texture,and date of application. Number of samples taken is determined by the size of the area that the client wishes to remove. The results outlined in this report apply ONLY to the activities performed on the date indicated and ONLY to the samples taken at the time. Once these materials have been removed and replaced with new materials additional testing would have to be performed to remove any materials at a later date. The results contained in this report apply only to the actual building materials sampled and there may be other asbestos containing materials somewhere else in the residence.This report has been prepared exclusively for the Client with specific guidelines as discussed in their scope of work. SEC does not warrant the work of regulatory agencies, laboratories or other third parties supplying information which may have been used in this report. Bulk samples are analyzed in accordance with USEPA test method EPA/600/M4-82-020 and EPA/600/R-93/116, which requires that distinctly different materiais or layers present within a sample be analyzed and reported separately. (For measurement uncertainty, refer to table 2.1 in EPA/600/R-93/116 method.) Sometimes it is not possible to completely separate thin or strongly adhering layers, such as paint or adhesive, and a combined result is given. For some complex materials, such as drywall/tape/mud joints, a composite or overall asbestos content may be reported in addition to individual layer results. RNET, Inc. is currently accredited for bulk asbestos analysis by the National Voluntary Laboratory Accreditation Program (NVLAP) of the National Institute of Standards and Technology (NIST). Our NVLAP laboratory code number is 200448-0. The analytical results provided in this report apply only to the samples actually submitted. This report is considered confidential. Details of this report will not be discussed with any person or agency not associated with you or your organization. This report must be reproduced in its entirety and shall not be copied in part or used by the client to claim product endorsement by NVLAP or any agency of the U.S. Government.
