No. of Pages Attached: Field Representative: Structural Steel Cover Report As scheduled, a representative arrived to perform special inspection services. The following is a summary of services provided on this date. June 12, 2014 Site 14-6534 As directed by Chad with Aspen Wireless: 1) GROUND inspected welded connections of plate to existing beam and plate to new beam. A summary below is of the following: A) It was observed that only one connection was placed on each end of the beam web, rather than the two on each side of the web, discrepancy item #1. B) It was observed that plates were used in lieu of 4x4 angle or bent plate. GROUND is requesting clarification from the EOR. Picture #1 attached to the second page of this report, discrepancy item #2. 2) GROUND inspected torched cut beams and found the beams had no Reentrant (radius) corners as per AWS D1.1-10 section 5.16, gouges greater than allowed per AWS D1.1-10 section 5.15.4.4 and slag not removed. Picture #2 attached to the second page of this report, discrepancy item #3. 3) in regards to note #2 on page SP-4 and of notes on pages, GROUND was not provided continuous observation while depositing of the welds, discrepancy item #4. 4) GROUND has received and reviewed Welding Procedure Specification (WPS), Welder Qualification Test Record (WQTR) for Thompson welding in accordance with AWS D1.1 and a continuity log. 5) It is noted on this date, the welds in place on todays inspection were found in general compliance with AWS D1.1-10 table 6.1; however, remain non-compliant per discrepancy item #1-4. June 16, 2014 Site 14-6534 As directed by Chad with Aspen Wireless: 1) GROUND re-inspected torched cut beams and found the beams to be in general compliance with AWS D1.1-10 clause 5, discrepancy item #3 is closed on this date. 2) GROUND has received and reviewed Approved stamped Town Of Vail updated detail SP-2 in regards to discrepancy item #1 and #4. These items closed on this date.
