Description of Work:
We propose to furnish labor and material for plumbing—complete in accordance with the below
specifications as follows:
• Please note that this proposal does not cover any existing problems with existing snowmelt
tubing at driveway—These issues are to be addressed separately
• All existing snowmelt piping at mechanical room to be cut back and clean in a professional
manner in order to facilitate adding snowmelt boiler at a later date
• 48 hours prior to demo, add a system cleaner and run system
• Immediately prior to demo,flush system until clear
• All glycol to be recycled properly
• Demo and remove existing boilers and necessary mechanical piping to facilitate new mech room
piping
• Install plywood at mech room wall and paint gray where necessary
• This will enable hanging boilers and all necessary piping
• Provide and install (1) 200,000 BTUH Energy Star rated Lochinvar Knight wall-hung modulating
condensing boiler
• Boiler to be piped and wired per manufacturer's specifications to enable maximum efficiency
• Boiler flue piping to be ran out through existing boiler flue
• Boiler flue piping to be Centrotherm Innoflue
• Provide and install 1 Caleffi air scoop, backflow preventer and fast fill
• Provide and install 1 HTX expansion tank—to be sized accordingly
• All pumps to be GrundFos with isolation flanges
• System pump to be GrundFos variable speed
• Remove and replace all zone valves in mechanical room with new Honeywell zone valves
• Provide and install 2 SuperStor 80 gallon domestic sidearm water heaters
• Water heaters to be piped and wired to allow for domestic priority
• Provide and install one GrundFos recirc pump with built in aquastat and timer
• Provide and install 2 glycol feeders—1 at snowmelt system and 1 at house system

• Provide and install 1 condensate neutralizer
• Flush new system piping
• This proposal includes all necessary gas piping
• Provide and install 30%glycol solution at house heating system
• Add 2 liters of Fernox boiler conditioner to system to maintain proper pH
• Please note that this proposal is based on the understanding that the existing system works
properly outside of the mechanical room
• This proposal includes all low voltage wiring, controls, and permit fees
This proposal includes a 24 month service of the boiler, as well as a 2 year parts and labor warranty
This proposal does not include any line voltage electrical work
This proposal does not include carbon monoxide detectors
General Material Specifications: Interior Work:All drain waste and vent lines to be Schedule 40 PVC
pipe. All water lines to be Type M copper. Gas pipe to be Schedule 40 black steel and corrugated
stainless steel (C55).
Excluded from this Proposal:
This Bid does not include any line voltage wiring, concrete cutting and removal, concrete replacement or
patching, all roof penetrations, all roof repairs, testing, abatement, or disposal of Icad paint, asbestos,
mold or other hazardous materials,site utilities,excavation, mounting under mount sink, and permit
fees to be billed on separate invoices. This Bid does not include carbon monoxide detectors,fixtures or
faucets. This bid does not include any tasks or materials not specified in the Description of Work as
stated above.
Price: The price for the above Description of Work is the sum of Twenty Seven
Thousand One Hundred Dollars($27,100.00).
Payment Terms: Payments will be made as follows:
Deposit of 50 percent(%)and Final Completion of 50 percent(%).
Unpaid balances after 15 days after each phase of completion will be charged an additional
administrative fee of 2 percent (%) per month. Unpaid balances after 60 days of each phase of
completion will be charged an additional interest fee of 8 percent (%) per annum, compounded
annually. In the event of collection for non-payment with or without suit, PSI shall be entitled to recover
all collection costs and attorneys'fees.

Additional Terms and Conditions:
1. Plumbing Systems Inc. is not responsible for the discovery of improper or faulty plumbing; rusted or
defective pipes; acids in drain systems; lines that are settled or broken; existing illegal conditions;
defective roofing; and/or defective electrical.
2. All material is guaranteed to be as specified. All work to be completed in a workman-like manner
according to standard practices.
3. The price is based on the scope of work listed above. Any additional work will be priced as a Change
Order.
4. The price is based on work being completed during normal business hours and does not account for
overtime hours. Any request by Client/Owner to complete work during overtime hours will be
priced as a Change Order.
5. All changes, additions, extras and/or deductions will be based on a fixed price and agreed to in
writing as a Change Order before being incorporated into the project for both parties' protection.
6. This proposal is good for thirty (30) days.
7. All agreements are contingent upon strikes, accidents, or delays beyond our control.
8. Owner to carry fire,tornado, and other necessary insurance.
9. The contractor will excavate all main line trenches for underground. If jackhammer and/or other
mechanical devices are required to break rock in order to install underground plumbing, an
additional charge may be added to cover the cost of the additional labor and/or rental of said
equipment.
10. Please note that all mechanical parts can and will fail. It is the responsibility of the homeowner or
building agent to protect his or herself from property damage by maintaining cold air sensors and/or
a flood mitigation device or both.

The above prices,specifications, and conditions are satisfactory and hereby accepted.You are
authorized to do work as specified. Payment will be made as outlined above.
