A few years ago my driveway was severely damaged during road and electrical repair on our cul-de-sac. The driveway was replaced, but the work was extremely poor. I replaced the driveway during the summer. I am very pleased with the workmanship. I was not aware at the time that approval by the town of Vail was required. I received a "red tag" and was made aware of my mistake. I sincerely apologize. I contacted Mr. Martin Haeberle who graciously explained the process to me. I am sending the requested forms as enclosures. Also, the required check for $20 is enclosed. Again, I apologize for not following the requirements set by the town of Vail.
