services were performed at times scheduled t�y Premi�r B��ifclers betiween C7ctober 11 and Oc#ak�er 31,

When scheduled, representatives of our office peric�dicaify nbserved #he installation of dr'rlled and
epoxied reinforcing sfeef 1 an�hars. The drilleci hr�les were ok�served for clepth and cliameter, ti�e type of '
epoxy used v�as noted as well as fh�size of reinforcing steel/anchor placed,
Based �n the final test results t4 date, field abservafions, RF1's received and other project
documentatian received from the design team, it is aur opinior� that the rnateriais observed far th�
epoxied reinforcinc� steeflanchors were in general accordance wiEh project plans, specifications, or
a�plicable RFR's. '
7he observations and materiais testir�g as inclicated above were performed periodicaliy to {�rovide data
to th� �wner, Contractor and Subcontractors to �ssist tf�ern in evaluating the general compfiance of fhe
materials andlor wark with the contract documents at the times af oar observation and testing. We did
not supervise, direct or hav� any control aver any of the Contractar's or 5ubcontraetor's work. We were
not respansible for the Contractar's or Subcontractor's schedijlinc� of oirr s�rvices, means, methods,
procedures, techniques, or seq�fences �f construction, nor responsibie for ensuring that any Contractor
or Subcontrac#or performed or completed the work in accordance wifh the contract documents or
construction industry standards. '
