Underground services shallbe in conduit (PVC) fuom the utili$ transformer to the electric meter, main
disconnect switch, and to the first electrical distribution circuit breaker panel.

TOWN OFVAIL ELECTRICAL PERJTIIT GUIDEUNES
All installaUons of qterior hot tubs or spa's require a DRB approval from planning. This application will
not be accepted without a copy of the DRB approval form attached (if applicable).
If this permit is for installation of an exterior hot tub or spa on a new elevated platform or deck over
30" above grade, you must also obtain a building permit.
If this permit is for installation of an exterior hot tub or spa on any existing deck or elevated platform,
a structural engineer must review the existing condition and veriry that it will support the added
concentrated load. Please provide a copy of the structural engineers wet stamped leter or drawing
with this application.
If this is a remodel in a multi-family building with a homeowners association, a letter of permission
from the association is required.
If this permit is for a commercial space, two (2) sets of stamped drawings are required. Electrical

Dear Mr. Du Bois:
Walsh Environmental Scientists and Engineers, LLC (WALSH) has cornpleted an asbestos irrspection of
the residential duplex structure locatecl at 3996 East l.upine Drive in Vail, Colorado. WATSH
collected sarnples of visible porcntially asbestos-containing materials (ACM) frorn the structure for
analysis. The inspection was completed prior to a planned renovation of the building. Following is a
sununary and lecommendations based on the sarnpling completed on August 31,2007.
Asbestos Sampliug
Asbcstos sampling was conducted by Faron L. Comption and Travis R. Brophy, WALSH asbestos
inspectors certified by ttre Colorado Departrnent of Public Health and Environment (CDpUE) and the
Environmcntal Protection Agency (EPA). Copies of the inspectors' certifications are attached. Tlris
survey was pcrfonncd in accoldance with CDPFIE regulations governing asbestos inspections.
A total of 29 bttlk samples of dryrvall with joint cornpound and rexture, sink undercoating, exterior shrcco,
roohng maerials, and tar paper were collected and submitted to an independent laboratory for aaalysis.'l'he sample locations are described on the attached sarnple data sheers and ihown on Figures I and 2.
Laboratory Analysis
Collected samples were analyzed by Reservoirs Environmenal Services, Inc. (NVLAp Accreditation No.
101896) o{ Denver, Colorado, using polarized-light rnicroscopy (PLM), a bulk sample analysis merhod
established by the National Voluntary Laboratory Accrcdiation Program (NVLA-P). Bulk asbestos
sarnples were analyzed using EPA Metirod 600R-93-116. While the EPA does not "certify,' laboratories,
analytical methods follow EPA's recommcnded protocols using a NVLAP-accredited laboratory.
Chain-of-Custody
Chain-of-custody fonns were useii to track the possession atrd transfer of the sarnples from the tirne of held
collection tluough laboratory analysis. The fonns contain the following: sample numbers, signanrre of
collector, date of collectiort, iclentification of sarnpled materials, requested laboiatory analysis, signatures
of individuals in custody of re samples, and record of possession. Copics of the chain-of-custody forms
are attached.
Laboratory Results
Laboratory results and a description of each uraterial sampied during the inspection are sununarized in the
following subsections. Copies of the analytical daa and the asbestos inspection forms are attached.

condition at the time of the inspection and covers approximately 8,500 squire feet. Sarnple analyscs

the kitchell arld hallway sinks was collccted for analysis. The undercoating was in goo'rl cond.ition on each
sink and covers a total of approximarcly six square feer. Sample analysis lOentinea the kitchen sink
unclercoating to contain no irsbestos and the hallway sink undercoating to conhin 8% chrysotile; therefore,
the kitclicn sink is not regulated and ttre hallway sinli is a regulated asbestos-containing unterial (RACM).

tlte kitchen and hallway sinks was collected for analysis. The undercoating was in good condition on eachsink and covers a lotal of approxinrately six square feet. Sample analysis ioeniifiea rhe undercoating
prcsenr on both sinks to contain g% chrysotile; therefore, both sinks are RACM.
E{terior Stucco (Homogeneous Area 06} - Three sanrples of the exterior stucco lucared above and/or
below various windows and doors on the exterior of both-units were collected for analysis. The stucco was
in good condition at the time of the inspection and covers approxirnately 200 square ieet, Sarnple analysis
identified the stucco to contain no detectable asbestos; therefore, the maierial is not regulated.

Conclusions aud Reconrmeldations
WALSH conducted au asbestos irupection of the structure located at 3996 East Lupine Drive in Vail,Colorado. WALSH collected satnples of visible potentially asbesros-conraining materials (ACM)
present in the structure for analysis, Sink undercoating was identified to be a regulat€d asbestos-
containing material.
Sink Undercoatilg - The unr:iercoating pres€nt on the hallway sink in the east unit and on the hallway and
kitchen sink in the west unit was identitied to contain asbestos. The sinks should be removed and disposed
as asbestos waste prior to conducting renovation acrivities that will affect the materials. The roral quantityof asbestos to be removed does not exceed tlre trigger levels as defined by the CDpHE; thirefore,
Regulation No. 8 does not apply to the project. However, removal of any ainount of asbestos requires
conrpliance witir the OSHA /sbeslos in Qnstruttion Standard located at 29 CFR 1926.110I. The
rcmoved materials must be disposed as non-friable asbestos waste at an appr.opriate facility.
