When is asbestos testinq required?
ANY building projecG disturbing more than these threshold levels of building materials require asbestos testing:
One- and Two-Family Dwellings: 30 square feet
All Others: 150 square feet
Asbestos testing results must be provided with your application for a building permit.
Tests which identifo POSfiVE results at more than 1olo require abatement by a State-certifled and VaiFregistered abate-
ment contractor. An asbestos abatement permit must be approved, and the clearance letter must be submitted to the
Town of Vail before the building permit will be issued

EXECUTIVE SUMMARY
On fune L9,2009, Mr. Steve Shurtliffof DS Consultihg Inc [DSC) performed a limited inspection and
asbestos sampling at 1881 Lions Ridge Loop, Unlt 40, Vail, Coloradq in order to identify potentially
hazardous friable and non-friable asbestos-containing materials (ACM) wtthin a portion of the above-
referenced, single family residence in which a renovation is scheduled. Mr. Shurtliff performed asbestos
bulk-sampling of dryrwall (surfacing material) in a toal of three (3) locadons within the main and upper
lwels of the residence (su Appendix A for sample luations). Atl three (3) bulk-samples nrcre anal;zed by
Reseroirs Envircnmenhl, Inc (NVLAP #101896) with all ttrree (3) laboratory rcsults being nondetect
for ACM (see Appendix C for laboraury results).
PRO]ECT OVERVIEW
L lntroductfon
A limited inspection and bulk-sampling for ACM was conducted at 1881 Lions Ridge Loop, Unit 40, Vail,
Colorado, by Mr. Shurtliff on |une 19, 2009, at the request of Mr. George Sink with GT Woodwor}s. Mr.
Shurtliff is a Colorado State Certified Building Inspector and has EPA Accreditation #15413 (see Appndix
B for certificate). The purpose of the inspection was to identify, sample and assess potentially hazardous
ftiable and non-friable ACM within the residence in which renovations are scheduled.
lI. Structurel Design
The struchrre is a wood-ftamed, multi-level, single-hmily residence with an attached garage.
tlt. Sampling and Analydcal Procedures
The inspection, assessment and sampling were conducted by an EPA and AHEM accredited Building
Inspector qualified by experience, education and training in the recognition of potential ACM and
approved bulk-sampling techniques. The asbestos bulk-sampling was conducted on suspectACM with a
limited number of bulk-samples being collected within the residence.
The inspectio& assessment and sampling were performed in accordance with Environmental Protection
Agency/AHEM recommended procedures. These procedures call for the vlsual inspecton of the area of
concern and tlre collection and analysis of representative bulhsamples of zuspect material.
Some minor destuctive sampling was conducted. Walls, columns and perimeter pipe chases were not
broken into in order to locate and quantify suspect ACM. It should be noted that additional ACM might be
located in these and other inaccessible areas.
Random bulk-samples, representative of the suspect asbe$os-conaining building materials (ACBM) of
each homogeneous area (HA), were collected according to the guidetines published as Environmental
Protecdon Agency (EPA) Final Rule: Tide II of the Toxic Substances Control Act fISCa), 15 USC, Sections
2641 through 2654 and in compliance with 40 CF& Part 763. Representative sampling is based on the
following criteria:
1. The distribution of the suspect material throughoutthe HA.
2. The suspectmaterial s phpicd characteristics and application.
3. Random samplingpatterns determined foreach HA"
Suspect materials sampled and analyzed should be considered to be representative of materials in eactr
HAif:
1. They exhibit similar physical characteristics; and
2, The application of the sampled material can be correlated to the application of un-sampled
material.
Bulk-samples collected were andlzed utilizing theEPXs M4hodIor the DeDtmination qf Asbutos in BuIk
Building il4anrials (EPA 600/R/116 luly, 1993) and the McCrone Research Institut€ s TieJ.stutos
Particle Atlas as methods references.
Analysls of tlte bulk-samples was performed on the "date reported," as listed in the bulk-sample analysis
report
IV. Notes on ReportFoluat
Suspect materials alike in appearance and application were sampled as HAs. Suspect materials were
divided into three classiftcations:
1. Surfacing materiah sprayed or troweled onto structural building member.
2. Thermal System Insulation (TSD: any type of pipe, boiler, tanlq or duct insulation.
3. Miscellaneous: other suspect materials, floor tile sheet vinylfinoleum, ceiling tiles, insulatiorL and
finishing materials.
Condition assessments were performed by the accredited inspector at tlte time of inspection. Condition
assessments are listed in the following section. Ratings of "good,n "damaged" and "significandy
damaged" ar€ meantto indicate the overall condition of the material.
A material in "good" condition has no visible damage or deterioration, or showing only very limited
damage or deterioration.
A material in 'damaged' condition has the following characteristics;
r The sutface is crumbling blistered, water-staind, gouge4 marred or otherwise abraded over less
than one.tenth of the surface if the damage is evenly distributed (onequarter if the damage is
localized).
Accumulation of powder, dust or debris similar in appearance to the suspect material on surfrces
beneath the material can be used as confirmatory evidence.
A material in "significantly damaged" condition has one or more of the following characteristics:
' The surface is crumbling or blistered over at least one-tentJr of the surface if the damage ls evenly
distributed (one-quarter if the damage is localized).
r One-tenth (one-quarter, if localized) of the material is hanging from the surface, deteriorated, or
showing adhesive failure.
. Water stains, gouges, or rnars are over at least one-tenth of the surface if the damage is evenly
distributed [one-quarter if the damage is locdized).
Accumulation of powder, dust or debris similar in appearance to the suspect material on surfaces
beneath the material can be used as confirmatory evidence.

Response-action recommendations for asbestos containing HAs are listed in the section VII.
Recommendations may be for more than one HA" if materials are alike. Recommendations are either
"general" or "immediate." An immediate recommendadon indicates an imminent hazard exists and
should be addressed as soon as possible. A general recommendation indicates no imminent hazard exists
and no further abatement activities are required for removal of the material.
V. lnspectorcomments
No ACM was identified during the bulk-sampling conducted at 1881 Lions Ridge Loop, Unit 40, Vail,
Colorado. A total of three [3) bulk-samples were collected from drywall in the main and upper levels of
the above-referenced residence. All three (3) bulk-samples tested non-detect for asbestos.
VI. Homogeneous Area Descr.iptions
The following section contains sampled HA descriptions and sample locations. Percent asbestos content
for each sample indicated can vary depending on sample locations, homogeneity of the materials, and
type of application. The following samples were collected from the slngle family residence at 1881 Lions
Ridge Loop, Unit 40, Vail, Colorado, on June 19, 2009. The quantities are approximations and are subject
to field verification:

Overhead services are not allowed in the Town of Vail.
Underground services shall be in conduit (PVC) from the utilitytransformer to the electric meter, main disconnect
switch, and to the first electrical distribution circuit breaker panel.
The main disconnect switch shdll be readily accessrble, and located next to the meter on the exterior wall of the
structure. All underground conduits are required to be inspected before back-filling the trench.
In multi-family dwelling units, no electrical wiring orfeeder cables shall pass from one unit to another. Common walls
and spaces are exempt.
NM Cable (Romex) can be usdd only in single and multi-family dwellings. Type NM cannot be used in any
building mixedwith Type A,B',E,F,H,I,M &S occupancies.
Aluminum conductors smallerthan size #8 are not permitted.
TOWN OF VAIL ELECTRICAL PERMIT GUIDELINES
All installations of exterior hot {ubs or spa's require a DRB approval from planning. This application will not be
accepted without a copy of the pRB approval form attached (if applicable).
lf this permit is for installation of pn exterior hot tub or spa on a new elevated platform or deck over 30" above grade,
you must also obtain a building permit.
lf this permit is for installation of an exterior hot tub or spa on any existing deck or elevated platform, a structural
engineer must review the existing condition and verify that it will support the added concentrated load. Please provide
a copy of the structural engineefs wet stamped letter or drawing with this application.
lf this is a remodel in a multi-famlly building with a homeowners association, a letterof permission from the association
is required.
