As requested, structral steel special insp€ction rms conduded at lhe above referenced projecl psr
the IBC 2003 Edition - Chaptrr 17: Stnrctural Tests and Special Inspection and project requirements. I
arrived on site at 10:30 AM. I met with you to review tbe Foject requiremen8 and specifications. The steel
erector for this project is Trinity Welding Inc. in Leadvile, Colorado. The welder qualification test r€ports
for welders on site were reviewed and observed to be acceptable with project specifications. Copies of
certifications are on file with Trinity $rslding. Slhile on site I received copies oftbree engineering
clrifications from the project stuctural e, gineer, I[s. Kimberly R McGhee with I(RM Consultants, Inc. in
Vail" CO. daleA 8n3,9/I2, and 9/19/07. Please refer to the attached reports for documentation.
Visual welding inspection was conducted on Unit #3, #5,  utilizing the following drawings
providedby KRM ConsulEnts, Inc.: ) Unit #3 - Sl & 52: Job t0606t6,2)Unit#5 - Sl, 52, & $: Jtb
#0606-17, and 3) Unit #8 - SI, 52, &S3: Job #0607-28. Inspection was conducted fiom the Garage Level to
the Roofl,svel on all unit liste4 wher€ applicable. All fillet welds inspected were observed to be as detailed
and in compliance with project specifications and submitted engineering modifications.
This concludes all required stuctural steel special inspection per chapter l? ad project
requirements by Thunder Mouatain Testing for the l)rolean Residence Remodel projects referenced. All
areas of inspection were observed to be in compliance with pmject specifications.

If this permit requires a Town of Vail Fire Deparfinent Approval, Engineer! (Public Works) review and approval,
a Planning Department review of Health Department review, and a review by the Building Departmen! the
estimated time for a total review Will take as long as three (3) week.
All commercial (large or small) and all multi-family permits will have to follow the above mentioned maximum
requirements. Residential and small projects should take a lesser amount of time. However, if residential or
smaller projecEs impact the various above mentioned departments with regard to necessary review, these
projects may also take three (3) Weet<s to review and approve.
Every attempt will be made by thid department to expedite this pennit as soon as possible.

How it rclates to Building Permits:
Fill out the atbched check list with the Building Permit Aoplication.
If yes was answered to any of the questions then a "Public Way" permit is required. You can plck up an
application at either Community Development located at 75 South Frontage Road or Public Work, located at
1309 Elkhorn Drive.
Notice sign-ofb for utility companies. ALL utilities must field verify (locate) respective utilities prior to signing
application. Some utjlity companies require up to 48 hours notice to schedule a locate.
A construction traffic control/staging plan must be prepared on a separate sheet of paper. An approved site plan
may also be used. This plan will show locations of all taffic control devices (signs, cones, etc.) and the work
zone, (area of construction, sbging, etc.). This plan will o<pire on November 1o and will need to be resubmitted
for consideration for approval through the winter. Be aware that your resubmission for winter may be denied
depending on the location of construction.
Sketch of work being performed must be submitted indicating dimensions (lengrth, width and depth of work).
This may be drawn on the traffic confol plan or a site plan for the job.
Submit completed application to the Public Works offce for review. If required, locates will be scheduled for the
Town of Vail electricians and irigation crew. The locates take place in the morning, but may require up to 48
hours to perform.
The Public Works Construction Inspector will review the application and approve or deny the permit. You will be
contacted as to the status and any requirements that may be needed. Most permits are released 48 hours of
being received, but please allow up to one (1) week to process.
As soon as the permit is approved, the Building Department will be notified, allowing the "Building Permif'to be
released. Please do not confuse the "Public Way Permit" with the "Building Permif'.
November 15th, A new Public Way Permit is required eadr year if work is not complete. Re-
application each November 15th does not mean an automatic renewal.

Unlawful deposits: Subject to subsection C thereof, it is unlawful for any person to litter, track or deposrt, or cause to
be littered, tracked or deposited, sand, gravel, rocks, mud, dirt, snow, ice, or any othdr debris or material upon any
street, sidewalk, alley or public place, or any portion thereof.
Notice; Abatement: The Director of Public Works may notify and require any person who violates or @uses another
to violate the provision of subsection A hereof, or who has in the Director's employment a persc,n who violates or
causes another to violate the same, top remove such sand, gnvel, rocks, mud, di$ snow, ice or any other debris or
material within twenty four (24) hours after receipt of said noUce by the Director of Public Works. In the event the
percon so notified does not comply with the notie within the period of time herein specified, the Director of Public
Works, or other authorized agent may ciluse any such sand, gravel, rock, mud, dit, snow, icg debrls or any other
material to be removed from any skeet or alley at the expense of the notified.
Summons and Penalty: As an alternative to the notice for removal provided In subsection B abovg any person
who violates or causes another to violate the samer may be issued a summons to appear before the Municipal Court of
the Torun for said violations, and upon being found guilty of a violation hereunder be punished as provided in Section
1-4-1 of this code.
Notice and Penatty: It is unlawful for any person to fail or refuse to comply with the notice of the Director of Public
Works as provided in subsection B hereof, and any such person shall, in addition to payment of the expense of removal
incurred by the Director of Public Work, as provided in subsection B hereof, upon being found guilty of a violation
hereunder, be punishable as provided in Section 14-1 of this Code. (1997 Code: Ordinance 5 (1979).

Information Handout
The Vail Village and {ionsHead Village are invaluable assets to the communig. Thousands of
people come to Vail edch year to sightsee, recreate, shop and to enjoy everything the Vail Valley
has to offer. The conimunity and merchants rely upon our guests to generate revenue. Our
summer/winter guests Sxpect a pleasant experience while in Vail.
To insure that all of thel proposed construction has as little negative impact on the community and
on our guest as possible, the Town of Vail finds it imperative to create and implement the Vail
Village and LionsHead Village Construction Hours Handout.
It would be irresponsiblB for the Town of Vail, the construction contractors and the merchants, to
not actively participate [o minimize the impact of construction in the Vail and LionsHead Village
areas.
The Vail Village Streetgcape Project will continue through 2006. lmprovements scheduled
for 2006 will be concelptrated along Gore Creek Drive west of the Children's Fountain, Gore
Creek Promenade and around the frontage of the Mill Creek Gourt Building. Additional
streetscape improvembnts are scheduled to begin along East Meadow Drive.
All construction projepts and/or remodels within the Vail Village Core Area should contact
Scott Bluhm, Town of fail Streetscape Coordinator, at970477-3418 for further information,

The following givens ard intended to provide the basic foundation by which construction will be
completed in the villageq:
r The public's health, qafety and welfare shall be honored at all times.
o Adequate pedestriad, loading/delivery, vehicle, and emergency vehicle access and circulation
shall be maintained.
r Roadways and pedeftrian walkways shall be kept clean and free of dirt and debris.
r All construction delivbries, equipment, tools and materials entering the Vail Village and
pedeshian areas muFt go through CheckPoint Charlie. All LionsHead construction activity
must go to the Vail Pplice Department for a parking permit. Do not use delivery zones for
construction parking;jrefer to approved staging plan or make other arrangements for parking.
r The hours of outside bonstruction activity shall be as follows:
- April 15 - June 2B & September 5 - November 17,7:Q0 a.m. until 7:00 p.m., seven days aweek. I
June 30 thru September 1, work must end by 4:00 p.m. on Fridays.
November 13 - April 15, 2007 work allowed within daylight hours only.
- Deliveries shall be restricted to the hours of 7:00 a.m. to 11:30 a.m. on Gore Creek Drive
and 7:00 a.m. to 8:30 a.m. on Bridge Street.
- Special construclion delivery permits must be requested in advance from Public Works in
conjunction with Qode Enforcement at Checkpoint Charlie.
- No constructiorl activity shall occur on Saturday or Sunday between June 26 and
September 3,

INTRODUCTION:
on March 12n,2007 an inspection/survey was conducted and 3 bulk
samples were collected from the:

Unit #5
400 E. Meadow Road
Vail, CO. 81657
The purpose of the inspection/survey was to locate and sample
suspected Asbestos containing materials that might be present in the area of
the Residence Unit that is planned for renovation.
The inspection was made, and the samples were collected by rohn R.
Peterman, an A.H.E.R.A. and State of Colorado Certified Asbestos
Inspector. Great care was taken during the inspection and sampling to be as
accurate as possible. It should be noted that minimal damage was done to
the existing building structures during the inspection so there is no
documentation for unseen conditions or stored items.
All samples were analyzed by DCM science Lab in wheatridge, co.
This laboratory is deemed "Proficient" in the E.p.A.euality Assurance (eA)
program for the determination of asbestos in bulk materials, and is
accredited by the American Hygiene Association (AHA).
SAMPLING PROTOCOL:
A random sampling scheme was used to sample the suspect materials that
were discovered. If during any future demolition or renovation work,
suspect material is discovered that hasn't been sampled and would be
disturbed, work should be halted until the material has been tested.

RENOVATION AREA DESCRIPTION:
The Residence at The Tyrolean, unit #5, is planned for selective renovation.
The interior walls are covered in sheetrock with a light texture, and the
ceilings are covered in sheetrock like the walls. The floor is covered. with
finished wood. The heating is provided by baseboard heating, and a
fireplace.
CONCLUSIONS & RECOMMENDATIONS :
Laboratory analysis, of the bulk samples, indicates that no Asbestos was
detected in the bulk samples.

Bulk samples are prepared for analysis using a 10X-80X stereo microscope in a hepa filter hood which providesa contamination-free environment. The sample is then analyzed by polarized light microscopy (pLM) at 100X.when the sample consists of.more than one layer, each layer is prliared and inalyzed sepblaiety. Fiber andmatrix materials are identified by the characterization of opiicat p'rop'erties inctudin!'color and pleothrorism,form, cleavage' relief, birefringence, extinction, orientation, twin;ini, interference igure and oiher distinguishingfg3tur99. Dispersion staining is also used to further aid in mineral identification. All percentages of asblstos,other fibers and non-fibrous constituents are calculated from the values obtained from the stereo and pLM

scales to 24" wide. Change the drawings to maintain the required minimum width in the extended

9 The extended balcony is required to have a ceiling height of 7'minimum. Provide a section to verifo

10 The elevations indicate there is an issue maintaining the required 78" head clearance at the top of
the relocated spiral stair. Move the stair and provide a cross section to veriff that the minimum code
