2. It appears that the existing conercte t)oor covering in the living room" kitchen. and entry has
been removed and a lightweight conuetc slab has been installed. From information prol'ided
it is our undersunding thst th€ concrete used firr thc new slab was G,rp-{)rete 20{J0 and that
the concretc cor,ering is 1-1/2" rhick.
3. l'rorn a rcview of the Cyp{rete rleb site, Gyp.Cretc 2000 is a light weight concre{c intended
for usc as a floor covcring over r'ood fltxrr systems.
4, It has been reported to this office that the original cortcrete floor cor,ing was rernoved because
it hed experienced significant cracking.
5. Wc observed no cracking in the nerr concrete lloor covering-
6. lt is our undetstanding lhat shetrock work had bcen complelcd prior to the installation of the
nell concrcte floor covcringl we oh*erved no sigrrilicanl dislrcsses in the sheetrrck on the llrst
floor ofthe unit.
7. Floor slopes were observed thraughout lhe unit. pa*icularly on the second floor; although it
app€ars thal the lloor slopes were likcly prcsent prior ttr the placemcnt ol'lhe new concretc
floor coverings since shqetrock distrcrses ft'crc not rrbsened.

Amendment to the 1999 N.E.C. Town of Vail Ordinance 10-l{.
Overhead services are not allowed in the Town of Vail.
Underground services have to be in conduit (PVC) from the transformer to the electric meter, main disconnect
switch and to the first electrical distribution circuit breaker panel.
The main disconnect switch shall be located next lo the meter on the exterior wall of the structure easily
accessible.
e All underground conduits are required to be inspected before back-filling the trench.
e In multi-family dwelling units, no electrical wiring or feeder cables shall pass from one unit to another. Common
walls and spaces are accepted.e NM Cable (Romex) is not allowed in commercial buildings or structures exceeding three (3) stories.
e No use of aluminum wire smaller than size #8 will be oermitted with the Town of Vail.
TOWN OF VAIL ELECTRICAL PERMIT GUIDELINES
e All installations of exterior hot tubs or spa's require a DRB approval from planning. This applicatbn will
not be accepted without a copy of the DRB approval form attached (if applicable).
e lf this permit is for installation of an exterior hot tub or spa on a new elevated platform or deck over 30"
above grade, you must also obtain a building permit.
e lf this permit is for installation of an exterior hot tub or spa on any existing deck or elevated platform, a
structural engineer must review the existing condition and verify that it will support the added
concentrated load. Please provide a copy of the structural engineers wet stamped letter or drawing
with this application.
e lf this is a remodel in a multi-family building with a homeowners association, a letterof permissionfuom
the association is required.
e lf this permit is for a commercial spaoe, two (2) sets of stramped drawings are required.
