Eco Tech Environmental, Inc. ("ETE") hereby proposes to provide you with the following services. Abatement will include the proper removal and disposal of the identified asbestos containing materials ("ACM") at the above n:ference<l Project. Estimated time and quantities are based on our own observations. The \X'ork will be <lone in strict accordance with all Environmental Protection Agency ("EPA"), Occupational Safety and Health ,\dministration ("OSHA") and Colorado Department of Public Health and Environment ("CDPHE") rules and regulations, as well as any specifications noted above. Scope of Work Work will include all labor, materials, fees, transportation and disposal of asbestos waste. ETE shall provide the Services and Deliverable(s) as follows: • Removal of sheet vinyl flooring in kitchen, approx. 150 square feet. Assumptions &Conditions • Normal work hours. • Required Power and Water provided by Owner. • Existing Permit to be amended from CDPHE to include unit N-3 • Clearance air monitoring is included in this pricing and will be performed by Sunrise Environmental. A.sbutos Abatement Propo.ralfor Town of Vail· Ju!y 11, 2014 1 1 Fee Schedule We propose hereby to furnish material and labor in accordance with above specifications and shall not exceed $ 3,890.00 unless otherwise agreed to by both parties via the project change control procedure, as outlined within . . \ PCR will be issued specifying the amended value. ETE reserves the right to withdraw this proposal if not accepted within 30 days. \Ve accept cash, check and all major credit cards'* as forms of payment Terms & Conditions All materials and labor are !,'llaranteed to be as specified. The Work will be completed in a workmanlike manner and in accordance with standard industry practices in the region. This Proposal is contingent upon strikes. accidents, regulatory actions or other delays beyond our control. The person signing this Proposal on behalf of Customer/Owner represents and warrants that he or she has authority to do so and that this Proposal, once signed, shall constitute a valid and binding contract \\ith Customer/Owner. For credit card transactions over SHl00.00 \\ith a credit card present \\ill endure a 2.75% fee. Project Change Control Procedure ThL· following process will be followed if a chani,>c ro this Proposal is required: . \ Project Change RL·yuest (l'C:R) "ill be the Ychicle for communicating change" The PCR must dL·scribe the change, the rationale for the change, and the effect the changt· will have on thL· project. The designated Project Manager of the requesting party (Contractor or Client) will re\iew the proposed change and determine whether to submit the request to the other party. Both Project Managers ..,,;u review the proposed change and approve it for further investii,<ation or reject it. Contractor and Client will mutually ai,>rec upon any charges for such investigation, if any. If the investigation is authoriicd, the Client Project Managers will sign the PCR, which will constitute approval for the investigation charges. Contractor will im·oice Client for any such charges. The im·estigation will determine the effoct that the implementation of the PCR will have on SOW price, schedule and other terms and conditions of the Agreement.
