Dear Mr. Guzik,
Based on my site observations, and with the exception of the underdrain system and galvanizing of
structural steel, I attest that the work described by the referenced drawings appears to have been
substantially completed and is in general conformance with the intent of the design drawing. The
underdrain system was not installed; it had been deleted by the Town of Vail. Likewise,
galvanizing of structural steel members was deleted by the Town of Vail, so the structural steel
was not galvanized.

STATUS:
Fabrication of steel supports far double tee repairs has been completed for tee stems 103 - 108.
Welding of vertical plates to the existing steel hanger plates was in work. Shop drawings for the
steel fabrications had not been submitted. The CMU wall at the west end of the room above the
boiler room had been removed for access.
OBSERVATIONS:
1. I spot-checked some dimensions on the fabricated assemblies. They appeared OK. The
assemblies are structural steel painted with gray primer; they had not been hot-dipped
galvanized. The bearing pads had not yet arrived on site.
2. The welding crew had cleaned the surfaces of the existing steel plates to receive welds to
bare metal. I requested the entire eXisting steel hangers be sand blasted to clean up all the
accumulated dirt and corrosion.
ACTION: Contractor to sandblast the existing steel hangers.
3. We reviewed the lifting operation. The lifts are tentatively scheduled for Tuesday, April
28. I requested at least 48 hour notice of the lift date.
ACTION: Contractor to confirm lift date with Randy as work progresses this week.
Randy to advise Fred of actual lift date.
4. I requested the welds be inspected prior to the installation of the new steel assemblies as
per the Quality Assurance notes on the drawings.
ACTION: Field welds to receive special inspection.

STATUS:
The steel supports for seven tee stem hanger assemblies were in place. The grout for the steel
plates on the deck above the hangers had been installed the day before.
OBSERVATIONS:
1. The steel hanger assemblies had been set in place but not lifted as of 8 AM. I
spot-checked the installations and all work appeared satisfactory. Some undercutting
was observed along a vertical weld at the new vertical plate to the existing hanger plate,
but it was considered acceptable.
2. The grout at the bearing plates above the tee stems was still soft as of 8 AM, after being
placed the afternoon before. I could remove some of it with finger pressure. I rejected the
grout. The grout was removed during the morning and replaced with Dayton Superior HD
50 Horizontal Repair Mortar in the early afternoon. This material set-up rapidly and the
bearing plates were reset. Lifting commenced about 3 PM.
3. Lifting was accomplished in the following sequence: #103, 104, 105, 106, 107, 108, and
179. The tee stems were lifted just enough to permit removal of the deteriorated pads and
the old shim stacks. All were then lowered a small amount to approximately the same
elevation where they were prior to the lifts. #103 may have been lowered slightly more to
try to line-up the deck surface above it and the deck to the west on the other side of the
expansion joint as best as possible.
4. The spacer plates shown on Details 4/S3 and 5/S4 were not installed prior to the lifts so
they could be installed snug against the tee stems after the lifts.

ACTION: Contractor to install the spacer plates.
5. The existing sire sprinkler pipe at hanger assembly at stem#179 was sprung about 2"to
make room for the hanger. While it appears OK in its current position at this time, I am
concerned that future movements of the tee may result in abrasion at the pipe surface,
weakening the pipe.
ACTION: Town of Vail to evaluate the pipe and determine possible remedial action.
6. Randy, Ed, and I observed the corroded rods at the previous hanger repair at stem 117 on
Leve13. These corroded rods are to be replaced. That hanger will need to be shored and
reshored to the lowest (slab-on-grade) level below prior to rod removal.
ACTION A: Contractor to install sharing here. The design shoring load should be 26
kips to level2 and 52 kips to level 1. Parking should be prohibited on the tee and the tees
immediately adjacent to it during the rod removal and replacement operation.
ACTION B: Because the replacement rods and nuts are now not galvanized, they
should be thoroughly cleaned, primed and painted prior to installation and touch-up
painted after installation, this to guard against future corrosion. The presumed source of
water that caused a major part of the existing corrosion appears to have come from the
failed seals around the boiler flue enclosures to the north. The seals in these regions
should be repaired.
7. Randy, Jeff, and I reviewed the upcoming column repair at level 1 and the upcoming
concrete topping removal and replacement at level4. I prepared sketches (attached) to
illustrate the required rebar for the topping.
ACTION: Contractor to install rebar per the attached sketches. J.R. Harris &
Company is to be notified for a rebar inspection prior to concrete placement.

STATUS:
Topping had been chipped off the top of the double tee flanges. The slab-on-grade panel adj acent
to the area of removed topping had been removed.
OBSERVATIONS:
1. Several holes through the tee flange had been left behind after the topping removal
operation. The largest hole was about 12"x 12"; the others were much smaller. Blazer
was installing board forms flush with the bottom of the tee flanges.
2. Concrete obstructions had been discovered on the north face of the wall. Two of them
appear to be fins, shown on the 1973 drawings as terminating about five feet down from the
top of wa1L The third and largest appears to be the remnant of a former ventilation shaft
that had been cut off and capped when the 1990 addition with the drive ramp was installed.
All interfere with the drain system as designed.
3. I was informed that Town of Vail had changed the drain design by eliminating the pipe to
the storm drain, eliminating the trench north of the wall, and installing gravel wrapped in
filter fabric laterals extending from the face of wall to the north about siX feet,with a lateral
located between the three concrete obstructions on the north face of the walL The laterals
had been buried by 10:00 AM when I arrived. I discussed my concern with John King by
telephone that this arrangement could introduce water into the subgrade below the
slab-on-grade and result in softening of that subgrade in the future. I was advised that the
Town had changed the design and that part of the basis for that decision was that future slab
removal and replacement if necessary was more cost effective than building the buried
drain pipe system. I requested Blazer compact the subgrade. Charlie said he had a
hand-held compactor to do that.
4. The PVC membranes were installed over the U-rebars and sealed around the U-rebars

with tape intended to splice seams in the PVC membrane. That work appeared
satisfactory. Strips of drain board, about 8"wide,were set in place between the U-rebars,
oriented N-S and extending about 2 feet to the north. I requested they be folded snug
against the north face of the wall and in contact with the subgrade to avoid voids below the
slab-on-grade.
5. I observed the rebar's at the topping slab. The#5 bars had been installed too high; Blazer
lowered the #5 bars while I was there.
6. Blazer installed a grid of#4 rebar at 12"o.c. each way approximately in the mid-height of
the 6" slab-on-grade portion.

STATUS:
Topping had been chipped off the top of the double tee flanges. The slab-on-grade panel adj acent
to the area of removed topping had been removed.
OBSERVATIONS:
1. The underside of the westernmost tee flange exhibited broken surfaces near the expansion
joint and the north walL View from above, the topping slab at this same area appeared
sound.
Comment: It appears that removal of the topping in May 2015 had cracked the tee
flange in this area but the cracks were not observed at that time. After four months of
traffic, sufficient flexing and vibration had occurred to result in the broken flange.
Action: Remove old wood that was attached adjacent to the expansion joint in former
years. Patch the underside of the flange. Submit a proposed material and procedure far
the patch.
2. The concrete fill over the north wall has several transverse cracks; the most severe cracks
were near the west and east ends of the concrete fill.
Comment: The damaged concrete will need to be removed and replaced. Randy
expressed the preference to defer those repairs until spring.
Action: For now,rout and install sealant at all the cracks—to keep water out, this to
minimize further damage until spring. JRH to prepare a design detail for repair in spring
2016.
