As requested, CTL � Thompson, Inc provided testing and observation services for The
Sebastian Penthouse remodel project in Vail, Colorado. Services were provided on a part-
time, on-call basis.
We performed reinforcing steel placement observation of the crane pad footing and
Penthouse slab on deck (see reports A1 and A2, dated July 22 and September 11, 2015).
Our observations indicated reinforcement was placed in general conformance with the
approved drawings and standard construction practice.
We performed flow fill testing of the subexcavation below the crane pad footing
(Sample No. 2769) and concrete testing of the crane pad footing and Penthouse slab on
deck (Sample Nos. 2923 and 5865). The test results indicated the average 28 day
compressive strengths achieved specified strengths.
We performed weld and bolt observations presented in our Report Nos. 1 through 7
dated July 23 through November 4, 2014. Our observations indicated welds and bolts were
in general accordance with quality standards and were in general conformance with
structural drawings.
We performed sprayed fire-resistive materials observations presented in our Report
Nos. 1 through 3 dated September 14 through December 9, 2015 and Progress Report No. 1
dated September 23, 2015. Our observations indicated materials were placed in general
conformance with project plans and specifications.
