General Information: This application is required for all proposals involving minor changes to buildings and site improve-
ments, such as roofing, painting, window additions, landscaping, fences, retaining walls, etc. Applicable Vail Town
Code sections can be found at www.vailqov.com under Vail Information —Town Code Online. All projects requiring de-
sign review must receive approval prior to submitting a building permit application. An application for Design Review
cannot be accepted until all required information is received by the Community Development Department, as outlined in
the submittal requirements. The project may also need to be reviewed by the Town Council and/or the Planning and
Environmental Commission. Design review approval expires one year from the date of approval, unless a building per-
mit is issued and construction commences.

This letter has been sent to you on behalf of AT&T Wireless (AT&T) which has a wireless
communication facility located at the above referenced address. As a result of new technology,AT&T
needs to modify its equipment on the existing site.
This activity involves removing(1)antenna and replacing with(I)new LTE antenna.Adding(2)
new LTE antennas. Adding(3)new RRH behind existing screen wall.Adding(2)new surge
suppressor behind the existing screen wall.Inside the equipment room AT&T will be removing(3)
TDMA cabinets,Removing(1) GSM cabinet,removing(2)existing battery stacks and replacing
with(3)new battery racks. I have included drawings for your review.
Consistent with the agreements governing this site, we are sending you a letter to document your
consent. Please return the attached Confirmation of Letter of Authorization via mail to our office
located at 1100 W. Littleton Blvd, Suite 410D,Littleton, CO 80120 or via email to perrv.carroll@inte-
grat-ed.com. Please include a contact number where you can be reached and fill in any special
instructions,so that we can inform you once a construction schedule has been established.
