Frontage Road West Vail, CO 81657 RE: P~•att Residence Addition JVA Job #12770 Dear Henry: ~-~ ~~~ ~~ r r ~~~ ~ I visited the site on l~lovember 9, 2007 to observe the repairs that were noted as being required in my Observation Report #l dated October l7, 2007, The following repairs have been made and any additional modification is noted: Sheathing has been added to the girder truss as required. The Contractor has added it full length rather than the 6'-0 length we showed. This is acceptable. The sheathing was to have been nailed to all framing members behind it with 16d nails at 4" on center. The nailing in place did not meet this requirement. Nails were to be added. ~Ve do not fee( that another visit is required to see this nailing. It wil! suffice if either you or the Contractor will send photos of the properly nailed sheathing. Please include a measuring tape in at (east one photo to give me a reference to compare the spacing. 2. The 118" bent steel plate has been added to the inside of the spliced LVL beam on the west dormer. Two of the screws apparently snapped off during installation so that those two screws are missing. This is acceptable as is the overall installation of this plate. 3. Plywood gusset plates have been added to both the trusses with field- trimmed ends that I noted in my report and on the trusses where the improper snow load was used in the design. The Truss Manufacturer has provided these truss repairs directly to the Contracror. I questioned the fact that fairly symmetric trusses had repairs at only one end or different repairs at the two ends. The Contractor provided me with copies of the repair drawings provided by the supplier. While the repairs are only shown at one end, I recommended that the Contractor confirm this with the supplier. Framing for the walls and roof was in place. Several areas of framing were not installed per the plans, especially at the west dormer. Rather than using ladder framing running east/wes# from the valley beams and extending out over the west wall as shown on the plans, the contractor has installed a ridge beam with standard rafters running down the slope to the valley beams and short outrigger ladder framing for the overhang. This changes the load paths. The connection far the ridge beam and the valley beams to the girder truss consist of a hanger at the ridge and toe nailing at the valleys. This appears to be acceptable except that the ridge beam did not line up with the vertical web member of the girder truss. The contractor has installed solid wood blocking in the voids of the truss to accommodate this condition, but further repair is necessary. See the attached red lined picture for the required repair, which consists of adding sheathing to the face of the truss. 2. The above referenced girder truss has a smaller top chord than bottom chord. Henry Pratt has confirmed that this is as shown on the truss shop drawings. The top chord is made up of 2x4's. This presented a connection problem with depth of the rafters being deeper than the chord. The contractor has added a 2x10 nailed to the chord and the rafters framing to this added member. This is acceptable as long as the nails used were at least 16d nails spaced at 6" or less. 3. The plans called for a truss over the north dormer window with a dropped top chard to allow the ladder framing to run over. The contractor has installed a "bent LVL" beam that consists of three LVL's . Twa of the sloped LVL's from one side lap with one from the opposite side connected with nails. This is not adequate. To repair this condition a steel plate shall be added to the existing header. See the ~<~ached red lined picture. 4. The ends of trusses have been roughly trimmed at the east face of the bearing wall between the bathroom and the main room of the space. On at least one truss, the connector plate between wood members was actually cut in addition to the vertical wood member. The truss manufacturer shall be contacted in regards to this issue for their approval or required repair. This is their design, and requires their approval. 5. The contractor has framed the low sloped section of roof adjacent to the existing turret in a manner other than that shown on the plans. A short beam spanning north/south between the existing wall and the north wall of the closet. A header over the closet door picks up this beam. The posts that support the header bear on the floor sheathing. The contractor shall verify that these posts bear directly over floor joists. If they do not, blocking shall be added below these posts between the adjacent floor joists. The contractor shall also verify that floor joist spacing in this area was reduced to 12" as shown on the plans. If it was not, additional joists shall be added between the existing joists. 6. All of these issues are due to the contractor making changes without consulting JVA. No changes should be made to the structure without prior approval from JVA. We are willing to discuss options, but need to be involved in the decision rather than trying to come up with repairs which takes more of our time and will take more of the contractor's time to install.
