No. of Pages Attached: Field Representative: Structural Steel Cover Report As scheduled, a representative arrived to perform special inspection services. The following is a summary of services provided on this date. March 31, 2015 Site 15-6516 As directed by Bobby Henderson with Vail Custom Builders, GROUND inspected the following: 1) Inspected welded connection of HSS column cap plate to bottom of W14x120 near grid C.7/5.2 of the upper framing. This connection was bolted per detail 2/S4, however; as informed the bolts would obstruct the finish of the dry wall. This connection is now welded on two sides with a butt weld and 3/16" fillet and the holes filled with weld. This will need the Engineer of Record approval. Pictures are attached to the second page of this report and is added as discrepancy item #1. The welds are found visually in compliance with AWS D1.1-10 clause 6 table 6.1, however, until the required documents are received as requested in below item #2, this connection are non-compliant. 2) GROUND is requesting Welding procedure specification (WPS), Welder Qualification Test Record (WQTR) for AWS D1.1 and continuity logs if WQTR's are not qualified within the last 6 months from AG Welding. This will be added as Discrepancy item #2. 3) It was brought to the attention of Bobby with VCB, as requested from the Town of Vail (TOV), RFI's and project plans need to be approved by the TOV prior to GROUND signing off on the projects work or open discrepancies. This connection was found in compliance with revised detail SK2 dated 4/2/15; however, this will be discrepancy item #3 for approval from the Town of Vail. The welds are found visually in compliance with AWS D1.1-10 clause 6 table 6.1. 2) GROUND has received and reviewed Welding procedure specification (WPS), Welder Qualification Test Record (WQTR) for AWS D1.1 and continuity logs from AG Welding. Discrepancy item #2 is closed on this date.
