Mueller, Owner/Builder Unit 405, Phase I, The Lodge at Lionshead 380 E. Lionshead Circle, Vail CO Refer to related drawings A1.0, A1.1, A2.0 and A3.0 for detail information. This Condominium Interior Remodel Consists of: 1. Fireplace Replacement. 1A. Removal of wood-burning fireplace (which has an existing gas log insert) and surround and flue ducting in chimney, and removal of related soffit in living room, upper level. The gas piping for the old gas log will remain and be reused for the new gas fireplace. A 15" raised platform will be installed, on which a new firebox will be located, as will framing for a new surround. This work will be done by the owner/builder under this permit. 1B. Installation of a new gas firebox (Mendota Model DXV35), including flue venting, will be by Colorado Comfort Products, 255 Wyandot, Denver. The existing building exterior chimney will be used, but CCP will be installing double-walled flue ducting for the new firebox. They will be obtaining a separate mechanical permit for this installation. 1C. The final framing for the fireplace surround, plywood or cement board backing, and installation of facing stone will be by the owner/builder and/or by a stone mason under his immediate supervision. The fireplace manufacturer's specifications will be followed regarding clearances for combustibles in this area. This work is being done under this permit. 1D. In conjunction with the above, electrical wiring for a timer and a 120VAC receptacle for the blower in the fireplace will be installed by the owner/builder, obtaining power from an existing electrical receptacle box that will be now contained behind the new surround. Anew receptacle will be added in the facing of the surround to replace the old one. An existing light switch for the outdoor porch light will be relocated into the surround. A new switch will also be located in the surround, and from it conduit will feed power to a new interior track lighting strip. This new track lighting strip will be located adjacent to the fireplace surround, on the wall above the glass doors that provide access to the porch. This work will all be done by the owner/builder under a separate electrical permit (see 2D below), application for which is also being made at this time. 2. Washer/Dryer Installation 2A. A new stacked all-electric washer/dryer will be added into one of the closets on the lower level. Manufacturer's specifications will be followed for all required hookups (plumbing, ducting, electrical). 2B. This will require the installation of new water supply and drain lines. These lines will be tied into the supply and drain lines in a bathroom, which is immediately adjacent to the closet. This work will be done by Sundance Plumbing from Avon or others, who will obtain a separate plumbing permit for this work. 2C. The exhaust venting for the dryer was installed last year as a part of last summer's major building renovation, and consists of a vent on the exterior of the building and related exterior wall penetrations. The owner/building will be connecting this existing exhaust duct to the new washer/dryer, per manufacturer's specifications, under this permit. 2D. The electrical supply for this washer/dryer will be provided by adding a conduit to an existing main floor receptacle box and running through the closet walls; a new 240VAC, 30Amp receptacle will then be added in the wall behind the new washer/dryer. The existing main floor receptacle box has its existing conduit routed directly to the circuit breaker panel. New wires will be pulled through that conduit and the new conduit. All work will be done in accordance with the 2005 edition of the NEC. This work will all be done by the owner/builder under a separate electrical permit, application for which is also being made at this time. 3. Stairway Wrought Iron Railings 3A. The existing wood railings and handrail at the interior stairway between the two levels of the condominium will be replaced by a welded wrought iron system. Details of this design can be found on the drawings. Final selection of the local craftsman to fabricate the welded sections has not yet been determined. All related Building Codes will be followed in the fabrication and installation of these items. All demolition of existing railings and preparation of the area to accept the new railings will be done by the owner/builder under this permit. Installation of the new railings will be by both the fabricator and the owner/builder. NOTE: Asbestos: The entire Lodge at Lionshead building was tested for asbestos prior to our last summer's major building renovation and sprinkler installation. This report concluded that Phase I "tested positive for Asbestos in the spray on ceiling texture" ... "in excess of the Regulatory Limit (Greater than 1~ Asbestos)". Also, "...The composite sheetrock samples ... contain Asbestos that is less than the Regulatory Limit. ... The composite sheetrock is not considered to be Asbestos containing and need not be abated prior to disturbance." (Ref, page 5 of Report). Peterman Asbestos Testing, Project Design, and Consulting INTRODUCTION: On Apri122nd, 2004, an inspection survey was conducted and 30 bulk samples were collected from the: The Lodge at LionsHead 380 E. LionsHead Circle Various Units and Common Areas in Phases I, II, and III Vail, CO. 81658 The purpose of the inspection/survey was to locate and sample suspected asbestos containing materials that might be present in the designated area of the unit that is planned for renovation. The inspection was made, and the samples were collected by John R. Peterman, an A.H.E.R.A. and State of Colorado Certified Asbestos Inspector. Great care was taken during the inspection and sampling to be as accurate as possible. It should be noted that minimal damage was done to the existing building structures during the inspection so there is no documentation for unseen conditions or stored items. All samples were analyzed by DCM Science Lab in Wheatridge, CO. This laboratory is deemed "Proficient" in the E.P.A.Quality Assurance (QA) program for the determination of asbestos in bulk materials, and is accredited by the American Hygiene Association (AHA). SAMPLING PROTOCOL: A random sampling scheme was used to sample the suspect materials that were discovered. If during any future demolition or renovation work, suspect material is discovered that hasn't been sampled and would be disturbed, work should be halted until the material has been tested. LionsHead Circle Vail, CO. 81658 RENOVATION AREA DESCRIPTION: The Lodge at LionsHead consists of three buildings that were constructed at different dates. Phase I built in 1970-1971, Phase II built in 1974, and Phase III built in 1978. The buildings are five stories tall with parking areas in the basement area. For the purposes of this report/inspection, each building is considered a separate homogeneous area. ~ Representative of LionsHead Lodge assisted in the inspection by escorting, and providing access and building information. The exterior walls are concrete with a rough texture applied to the outside. The interior of the building has had some renovation throughout the years. The original areas that have not been renovated were the focus of this inspection. The interior walls are sheetrock with a light texture. In some areas it appears that a troweled on texture has been applied over the original sheetrock and light texture. The ceilings in Phases I & II are sheetrock with texture like the walls and a spray on texture on the concrete areas and some sheetrock areas. Care should be taken in the sheetrock ceiling area to check for the spray on texture on the concrete above the sheetrock. Phase III ceilings are sheetrock with light texture like the walls. No acoustic spray on texture was observed in Phase III. The parking garage in Phase III has a concrete ceiling with a spray on fireproofing. The floors are covered with carpet for the most part; also some ceramic tiles. There is some sheet vinyl flooring that was not tested, as it will not be disturbed by the planned activities. The heating is provided by hot water baseboard heating and fireplaces. No visible suspect T.S.I. was observed in the units. The boiler rooms were not inspected at the time of this inspection. 4 A & D Asbestos Testing and Consulting John R. LionsHead Circle Vail, CO. 81658 CONCLUSIONS & RECOMMENDATIONSo Laboratory Analysis of the bulk samples indicated that Phases I and II tested positive for Asbestos in the spray on ceiling texture, and in the composite sheetrock samples. Exterior wall texture on Phase I also contains Asbestos. The spray on ceiling texture in Phases I and II contain Asbestos in excess of the Regulatory Limit (Greater than 1 °Io Asbestos). The exterior wall texture on Phase I also contains Asbestos in excess of the Regulatory Limit. These Asbestos containing materials must be handled as per Colo. Reg. #8 prior to disturbance. The composite sheetrock samples in Phases I and II contain Asbestos that is less than the Regulatory Linut. The Asbestos content of the sheetrock mud was verified by the required Point Count Analysis. The composite sheetrock is not considered to be Asbestos containing and need not be abated prior to disturbance. The O.S.H.A. regulations that protect workers from air borne Asbestos fibers are still in place. No Asbestos was found in Phase III. Bulk samples are prepared for analysis using a 10X-80X stereo microscope in a hepa filter hood which provides a contamination-free environment. The sample is then analyzed by polarized light microscopy (PLM) at 100X. When the sample consists of more than one layer, each layer is prepared and analyzed separately. Fiber and matrix materials are identified by the characterization of optical properties including color and pleochrorism, form, cleavage, relief, birefringence, extinction, orientation, twinning, interference figure and other distinguishing features. Dispersion staining is also used to further aid in mineral identification. All percentages of asbestos, other fibers and non-fibrous constituents are calculated from the values obtained from the stereo and PLM microscopes analysis. In-house and NIST standards as well as a chart prepared by R.D. Terry and G.V. Chilinger for "The Journal of Sedimentary Petrology", (Volume 24, pp. 229-234, 1955) provide a guide for estimating percentages.
