The following is a summary of services provided on this date. October 2, 2014 Site 14-6581 As directed by Dale with GE Johnson: 1) GROUND received and reviewed Welding Procedure Specification (WPS), Welder Qualification Test Records (WQTR) and continuity logs in accordance with AWS D1.1 for GE Johnson welding erectors. 2) GROUND inspected welded connections of Attic framing on grids A2-B/14-15 of HSS column to cap and base plates, HSS column cap plate to bottom of beam, HSS column cap plate to bottom of HSS, HSS to HSS and bolted connections of beam to beam. These connections were found in compliance with project plans, in general compliance with AWS D1.1-10 clause 6 and Bolts found in the "Snug Tight" condition. 3) GROUND is requesting written approval from the Engineer of Record (EOR) for adding column base plates and anchoring them to the foundation in lieu of welding to existing beams at the attic framing HSS columns, Discrepancy item #1. 4) GROUND inspected welded connections of elevation bracing at grids A1.6/13-13.8 and B/13-13.8 of Horizontal HSS to existing HSS column, horizontal HSS to Vertical HSS, Vertical HSS to slopped HSS. Below is of the inspection and requiring further information: A) there are plates connecting the HSS to HSS connections of both brace elevations. GROUND is requesting clarification from the EOR , discrepancy item #2. Picture #1 and #2 attached to the second page of this report. B) GROUND was not provided the opportunity to inspect the Vertical HSS to Existing wide flange beam at elevation "A" in foundation before covered with concrete, discrepancy item #3. C) slopped HSS was cut in the field and at elevation "A" peak it was found welded three sides with a gap at the top (not welded) of 3/8". GROUND is requesting clarification from the EOR, Picture #3 attached to third page of daily report, discrepancy item #4. D) after connections were repaired while GROUND was on site the Remaining connections was found in compliance with project plans and in general compliance with AWS D1.1-10 clause 6. October 9, 2014 Site 14-6581 As directed by Dale with GE Johnson: 1) GROUND received RFC #11 in regards to slopped HSS at the concrete walls; however, GROUND is requesting a stamped approved copy form the Town of Vail, discrepancy item #5. 2) GROUND is requesting from the Town of Vail (TOV) the use of approved Engineer of Record (EOR) shop details, discrepancy item #6. 3) GROUND inspected welded connections of braced frames elevation "B"of angle clips to beams, angle clips to HSS columns, 10x5 HSS to existing HSS columns, plates to existing and new HSS and HSS column to existing foundation beam. At both elevation "B" grid B/13 and B/ 13.7, the plates did not extend through the two existing columns as detailed in B/S201, GROUND is requesting clarification from the EOR, discrepancy item #7 and picture #1 is attached to the second page of this report. Remaining connections were found in compliance with Approved EOR shop details and RFC #11; however, these connections will remain non-compliant until GROUND receives answer for the use of RFC #11 and Shop details from the TOV. These connections were in general compliance with AWS D1.1-10 clause 6. October 21, 2014 Site 14-6581 As directed by Dale with GE Johnson: 1) GROUND received observation Report dated October 10, 2014 from Anderson & Hastings Consultants, Inc. with SX-5, SX-6 and SX-7. Until the observation report has been reviewed and approved by the Town of Vail (TOV), these connections are considered non-compliant, discrepancy item #8. The following below is of the inspection results: A) SX-5, discrepancy item #2. -Detail at vertical HSS column to Sloped HSS beam was found in compliance with the detail provided. -Detail at sloped HSS beam connection northeast corner grid B/13.7 was found with the added HSS not welded on top nor the bottom and with the plate still installed. This is not per details provided and this will be added as discrepancy item #9. -Detail at sloped HSS beam connection southeast corner on grid A1.6/13.7 was found in compliance with detail provided. B) SX-6, discrepancy item #7 and #2 -Moment connection at grid B of two locations. The east connection was found in compliance with provided detail. The west connection was found with the back side vertical plate not having a PartIcal Joint Penetration (PJP) weld and the front side had a filler rod in between the two plates, GROUND is requesting clarification from the Engineer of Record (EOR), picture #1 and #2 is attached to the second page of this report, discrepancy item #10. Bottom plate to HSS not completed, picture #2 is attached to the second page of this report, discrepancy item #11. -Detail at sloped HSS beam connection northwest and southwest corners grids 13 at A1.6 & B.1 was found in compliance with provided detail. C) SX-7 detail was found that the welded L6x4 in compliance with provided detail. D) the welded connections were found visually in compliance with AWS D1,1-10 clause 6. 2) GROUND was informed that a letter of compliance will be sent to the TOV for approval for the welded connections and details from the EOR. October 29, 2014 Site 14-6581 1) GROUND arrived on site and contacted Dale with GE Johnson. 2) GROUND was informed that a letter from the Engineer of record has been received by Dale in regards to Outstanding Steel Discrepancies and that it is in the process of being reviewed by the Town of Vail for approval. 3) GROUND re-inspected welded connection of moment plate to HSS column in regards to discrepancy item #11. This connection was found completed per project plans and in general compliance with AWS D1.1-10 clause 6. Discrepancy item #11 is closed on this date. Dale was present for the inspections. The following is a summary of services provided on this date. November 14, 2014 14-6581 1) GROUND received and reviewed approved Town of Vail (TOV) detail S-201 and S-302 details. A letter dated October 28, 2014 from Anderson and Hastings with attached observation reports (9/12/14, 10/9/14) was received on October 30, 2014 from Dale Zerr with GE Johnson. The following below is the response per each discrepancy item and the results: A) discrepancy item #1: A&H letter dated 10/28/14 accepts this condition of bolting in lieu of welding to existing beam with RFC #20; however, the detail was not added to the Approved details, no TOV approval of letter nor RFC #20. This item remains open. B) discrepancy item #2: details SX-5 and SX-6 have been added to TOV approved detail S-201, therefore this discrepancy is close on this date. C) discrepancy item #3: in regards to column not being inspected prior to covering, letter dated 10/28/14 states: "Per observation report dated September 12, 2014, it was noted that it is acceptable to weld the HSS columns to the top flange of the existing beam flanges found to be slightly off center, provided that the all-around weld as noted on the structural drawings could be achieved. Also, per response to RFC No. 008 the base connections were found acceptable at the north entry columns". This letter has no TOV approval, nor RFC #008. This item remains open. D) discrepancy item #4: in regards to slopped HSS was cut in the field and at elevation "A" peak it was found welded three sides with a gap at the top (not welded) of 3/8, per letter dated 10/28/14 states: "The connection reference was found to be acceptable per field observations dated October 9, 2014". This letter has no TOV approval. This item remains open. E) discrepancy item #5: GROUND has not received TOV approval for RFC #11 as requested, nor approved letter dated 10/28/14. These connections were found per detail and in compliance with AWS D1.1-10 clause 6 at the time of inspection. F) discrepancy item #7 & #8: SX-5, SX-6, and SX-7 was added to the TOV approved details, therefore these discrepancies are closed on this date. G) discrepancy item #9: noted on A&H letter dated 10/28/14 it appears as discrepancy item #8. Letter accepts the bottom and top weld not being in place; however, GROUND has not received TOV approval, nor approved letter dated 10/28/14. This discrepancy will remain open. H) discrepancy item #10: A&H required that the moment plate be welded as per detail B/S-201 grids "B" and SX-6 in letter dated 10/28/14. GROUND had received this letter on 10/30/14 from Dale and responded back with E-mail, informing Dale that this item needed correction per this letter. GROUND also arrived on site dated 11/12/14 and disused this with Dale. GROUND was informed that the connection was covered and A&H was notified. This item will remain open. 2) GROUND has not received TOV approval copies of EOR approved shop details in regards to discrepancy item #6. This item will remain open. 3) GROUND has talked with Dale and was informed that letters, revisions, RFI/RFC need Town Of Vail approval prior to GROUND signing off discrepancies or accepting the coditions. This has been applied to the details S-201 and S-302; however, the above discrepancies that remain open need approval. These connections were found approved and completed per project plans; therefore these discrepancies are closed on this date per the TOV approved letter dated 11/18/14.
