Dear Mr. Peterson:
Hepworth-Pawlak Geotechnical, Inc. provided construction observation and materials
testing services for foundation soil bearing conditions and structural backfill, reinforcing
steel, concrete and structural steel during construction of the referenced project. Our field
observation and testing services for the project began on December 22, 2014 and
continued on an as-requested basis through July 23, 2015. Our services for the project are
essentially complete at this time.
Our services were performed under the direction of the undersigned licensed Engineer
registered in the State of Colorado. Any nonconforming conditions, materials and/or
workmanship identified during construction were immediately brought to the attention of
the contractor for correction. Our testing and observation reports have been distributed
under separate cover.
Based on inferences drawn from our observations during construction and the results of
our testing as documented by the referenced field and laboratory reports, it is our
professional opinion that the referenced foundation soil bearing conditions, reinforcing
steel, concrete, structural backfill and structural steel for the project have achieved the

specified conditions, and/or currently accepted industry practices as applicable. Please
recognize that construction observation and testing are sampling functions that involve
direct observation and testing of very limited amount of the overall work to evaluate the
construction compliance.
