As requested I have outlined below our proposed activities, time frame, proof of insurance and additional assurances. Maximum Comfort Pool and Spa, Inc. (MCPS) respectfully requests from The Vail Corporation temporary access from Mill Creek Circle to the rear of the Tivoli Lodge according to the following conditions and as noted on the attached access plan. Description of Activities: 1.       Using a mini excavator we propose to remove the existing soil pile (approximately 30 cubic yards) from the spa excavation. 2.       Transport the soil off site from the excavation area using pickup truck and smaller dump trailer to limit compaction. 3.       Truck in gravel, road base and sand to complete the paver patio (approximately 10 cubic yards). 4.       Deliver tile and supplies to spa using pickup truck. 5.       Truck in topsoil and mulch to finish landscaping (approximately 8 cubic yards combined). 6.       Using mini excavator we will backfill  the spa and reconstruct the previously existing rockery and landscape using existing boulders. 7.       Clean up all debris and return park site to previous conditions. Time Frame: The above activities will be completed in the next two weeks between 12/11/2017 and 12/22/2017. Remediation Activities: 1.       Upon completion of the above tasks MCPS will clean up any debris and repair any damage attributed to this work. 2.       During the Spring season of 2018 we will additionally spread topsoil, seed or sod and any mulch necessary to restore to preexisting conditions. Insurance Coverage: MCPS shall provide to The Vail Corporation a certificate of insurance with coverage up to 2 million dollars naming The Vail Corporation additionally insured (see attached). Performance Bond: MCPS will post a performance bond in an amount mutually agreed upon by the parties. Please confirm the above meets with your approval. Thank you very much for your consideration.
