We are writing regarding the Asbestos Test performed by Strom Environmental, Inc. on June 19, 2007
at the above referenced address.
The above mentioned testing has concluded there is 25 square feet of floor sheeting that has asbestos
containing material (ACM). We have read the Asbestos Bulk Survey Report prepared by Strom
Environmental, Inc. and are aware of the location where the material exists which contains the ACM.
We are willing to live with the existing ACM and not have the removal and abatement of the material
perfomred at this time. We understand that the material will remain at the existing location and will be
covered with a new floor covering.

We have employed state-of-the-art practices to perform this evaluation, but this evaluation is
severely limited in scope to suspect asbestos-containing material easily accessible to a visual
inspection. No Demolition or product review was performed in attempts to reveal material
compositions. Our services consist of professional opinions and recommendations made in
accordance with Regulation 8 of the CDPHE and generally accepted engineering principles and
practices, and is designed to provide an analytical tool to assist the client. SEI or those representing
SEI bears no responsibility for the actual condition of the structure or safety of the site pertaining to
asbestos and/or asbestos contamination regardless of the actions taken by the client.
Note: This asbestos survey report is limited only to those areas listed.
PURPOSE AND SCOPE
The purpose of this investigation is to identify visually accessible asbestos-containing materials
throughout the accessible portions of the property located at 1750 S. Frontage Rd. West #D4, in
Vail, CO., which may present an asbestos exposure risk during renovation/demolition activities.
The project site consists of a three-story wood/brick-frame condominium structure situated in a
residential area. The components of the survey were:
-A site inspection to determine the presence and extent of ACM, including assessment of
the potential hazard posed by the ACM through visual inspection to determine the level of
material friability and to evaluate the accessibility of the materials.
-A bulk sampling program for potential ACM.
-Analysis of the samples utilizing polarized light microscopy(PLM) and Dispersion staining
Techniques. This procedure is accordance with USEPA Bulk Analysis Methodology,
USEPA 600/R-93/116.
-Development of recommendations for control options for confirmed ACM.

Observations and sampling results contained in this report are limited solely to the potential for
ACM in accessible areas of the site structures. The evaluation of concealed structural members
were not performed by SEI.
The asbestos containing materials(ACM) most likely to release asbestos fibers are those which are
in a friable state. Friability describes the condition of asbestos. The definition of friable is any
material which is capable of being crumbled, pulverized, or reduced to powder by hand pressure.
The evaluation is limited to those areas accessible for an inspection.
Non-friable sources of asbestos are the materials that have a cement or asphalt binder and may
become friable and release fibers if the sources are exposed to actions such as abrasion, drilling,
cutting, fracturing or hammering. Non-friable sources of asbestos do not typically pose a significant
health concern if they remain in good condition and are not disturbed. During demolition or
renovation activities, non-friable sources may become friable and thus may pose an exposure risk.
All areas that were accessible were tested for the presence of asbestos. At the time of the survey,
the construction drawings were not available.
State laws require that buildings owner(s) and/or their representative(s), prior to any demolition
and/or renovation operations that may disturb any asbestos-containing materials in their building(s),
must meet the following requirements:
* notifications
* removal techniques for asbestos-containing material(s)
* clean-up procedures, and
* waste storage and disposal requirements.
SURVEY METHODOLOGY
Prior to the collection of any samples, distinct homogeneous sampling areas and specific sampling
locations were based in accordance with Asbestos Hazard Emergency Response Act(AHERA)
protocol (40CFR part 763). A homogeneous sampling area is described as any material that is
identical in appearance, identical in color, and has the same installation date. Although the AHERA
regulations only apply to elementary and secondary schools and thus does not pertain specifically to
the property, the AHERA methodology generally is considered state-of the art for asbestos building
inspections.
Suspected ACM was identified by SEI through a physical survey. The physical survey involved the
inspection of the site structure and recording of locations, and conditions of suspect ACM.
Each type of suspect ACM was sampled randomly. Usually three samples per homogeneous
material were collected. Occasionally, the actual number of samples varied, dependent on the
professional judgment and experience of the inspector, as well as the type and amount of suspected
ACM.

ANALYTICAL METHODS
All bulk samples were analyzed by SEI, via polarized light microscopy utilizing dispersion staining
techniques according to USEPA Interim Method USEPA 600/R-93/116. All reference testing was
completed by an analyst qualified under the NVLAP Asbestos Bulk Sample Quality Assurance
Program(NIST/NVLAP Lab #200450-0).
The USEPA Interim Method requires the microscopist to take a portion of the bulk samples and
treat it with an oil of specific refractive index. This prepared slide is then subjected to a variety of
tests while being viewed under varying polarizations of light.
Each type of asbestos displays unique characteristics when subjected to these tests. Percentages of
the identified types of asbestos are determined by visual estimation. Any material that contains
greater than one(1) percent by area of any type of asbestos is considered an asbestos containin
material(ACM) and must be managed in accordance with CDPHE, OSHA, and USEPA regulations,
if disturbed.
SUMMARY OF RESULTS
After visual examination and analysis of samples taken from the property on June 17, 2008, it was
determined asbestos(>l%) was found in the following materials:
Floor Sheeting(tan)-l" floor laundry room(Approx. 25 SF)
*NOTE-The actual amount of ACM may vary due to limitations of visual estimations made by the
inspector, actual measurements will need to be taken to determine amount of ACM present.
All other sampled materials tested negative for the presence of asbestos. Refer to the analysis and
bulk sampling sheet included in Appendix A for a list of other materials sampled as well as the
condition of the materials tested.
*NOTE-Tar impregnated roofing materials, and window putty/glazing were not tested. If these
materials are rendered friable during demolition, these materials will need to be tested.
Comments- -This inspection is limited to visually accessible ACM suspect materials
sampled. If untested suspect materials are discovered during
renovation/demolition activities, further testing will be required before these
activities continue.

CONCLUSION
According to the visual investigation and bulk sampling of suspect materials, asbestos-containing
materials were used. It will be necessary to comply with the pertinent provisions of CDPHE,
OSHA, and EPA regulations during any renovation/demolition activities, which may disturb the
asbestos-containing materials.

Dear Building Official:
This letter is in reference to a proposed remodel of a unit in the Spruce Creek
Town Homes, D4 located at 1750 S. Frontage West, in Vail CO. The proposed
remodel includes removal of a portion of what is assumed to be a load bearing
wall on the second floor supporting the floor joists above. The wall will be
removed at the bath for a 6' maximum span opening that will require a minimum
of a 2-2x6 header. This header will be supported by a minimum of two 2x4 posts
at each end. These posts will bear on the existing floor support beam that should
be verified to be a 2-2x10 flush beam. See attached sketches for locations
referenced in this letter.
These beam sizes are based upon assumptions that the load bearing wall at the
bath on the second floor is actually load bearing and that this wall only supports
the third story floor loads and not any roof loads. All lumber should be Douglas-
Fir-Larch, No 2 or better and nailing shall conform to the 2003 IRC. Should field
conditions vary from what is described, please inform this office as soon as
possible so a new design solution can be worked out. We are willing to perform
any construction observations as necessary.

Enclosed is a check in the amount of $79.00 for a refund of the permit fees you
paid for permit M08-0190. This permit was for the conversion of a wood fireplace
to gas, and per Title 5-3-8 of our Town Code (Ordinance 21, 1991), all permit.
fees for wood to gas conversions are waived.
We apologize for our error originally charging for this permit, and have taken
steps to ensure this error doesn't happen again, by training staff, including a note
about the waived fees on our fee schedule, and updating our mechanical permit
application form.
