I arrived on site at 10:45 AM. I met with you to review the project requirements and specifications. The steel fabricator and erector for this project is Trinity Welding, Inc. in Leadville, Colorado. Trinity Welding Inc. fabricated all main member material on site prior to erection; therefore, Periodic inspection shall be as required as stated in Section 1704.2 in the IBC for non-approved fabricators. All column & beam material fabricated on site was inspected for proper fillet weld size, length, and location as indicated on supplied project drawings. All material fabricated prior to erection was inspected and observed to be acceptable with visual requirements stated in the AWS D1.1 -Structural Welding Code: Section 6 and project requirements. The welder qualification test reports and welding procedure specifications for welders on site were reviewed and observed to be acceptable with project specifications. Copies of certifications aze on file with you. While on site I received and reviewed a copy of a steel revision: Drawing S4: Detail 4 from the project structural engineer Mr. Thomas P. Skinner with JVA, Inc. in Boulder, Colorado dated 4/29/08. Please refer to the attached approved drawing for documentation. Visual welding inspection was conducted at the Upper Level Floor Plan and Roof Framing Plan on the field fillet welded column to beam connections, beam to beam fillet welded connections, angle bracing fillet welded connections, edge angle fillet welded connections, and metal decking puddle welds & side lap attachments as referenced on approved jobsitc drawings provided by JVA Consuhing Engineers: Job #12915 dated 1/7/08. Drawings utilized for inspection were located on Sheet S1, S4 (*Above referenced revised drawing), & S5. During inspection the following azeas were observed to be unacceptable with referenced drawings and are listed as follows: 1) Beam flange '/." stiffener plates as referenced on Sheet S5: Detail 1. I observed missing welds on all beam stiffener plates installed at the Roof Framing Plan Level -Grid line Y.4 - Y.8 & 16.2 -16.6, 2) Brace Angle (1/4") on revised detail referenced on Sheet S4: Detail 4 above. Fillet welds were not welded all the way around the connection faying surfaces at each end as required at the Upper Level Framing Plan Level -Grid line Y.4 - Y.8 & 16 -16.7, 3) Brace Angle (3/16") on Sheet S5: Detail 4.
