within a portion of � he sb�ve-reference�, single-family residence_ Mr. Brandon Sinkbezl
pei�furmed �.gbestns lrulk sampluig of various surfacang materials on shccttock in a total �f
three (3) locations within the resid�ncc. All thr'ee (3) butk samples wcre analyzed by
Reservoirs Environmental, Ine. with all laboratory resulls bcing non-detect for ACM (see
Appendix A for laboratory results).

An inspection aud bulk sampling for ACM was conducted at 4220 Spntcc Way in Vail,
Colorado, by Mr. Brandon Sinkbeil on May 7, 2008, �t ihc rcquest of C& H Conslructi�n,
LLC in Avon, CO. Mr. Sinkbeil is a Colorado Slate Cc�'tx�ied inspector, and has EPA
Accreditation #13442 (see Appenciix B for a c��py of tlus certificate). Thc purpose of the
inspection was to idcatlfy, sample and assess potenti�lly hazardous friable and non-&iAble
ACM in a portion of the single-�nmily residencc rcfcrcnccd above schedt,led for a
rein�deling.
II. Structnral Design

��. Sampling and Analytical Proccdures
The ins�eetion and assessmetit were conducted by an EPA and AHERA accreciite�l Inspector
qualified by experience, educ�tion and uai.nin� in the rec�gnition of poteniial ACM and
approved bulk s�wapling I.u;lnniqu�s. Th� r�.sUestos bullc szttnplin� was conducted on suspecl
AGM with a l�imited num�er of bulk samples being collected in the residence.
The inspecti�n and �i�sessment wer� nerformed in accordance with EnvironmentAl Protec:tion
�1,gency/AfiERA recpmmended procedures. Thcse procedures call for th�e visual insPection
of tlie area of coiicein and collection and aa�filysis of repre�en�tttive bulk samples of suspeet
matcrial.
Some minor destruclive sampling was conducted. Walls, columns and �erimeter pip� chases
wcrc not broken into in order to locate and quantify suspect ACM. [t sliould be notcd thAt
additional ACM might be localed in ol,hcr inaccessiUlc are�.s.
Raz�doztx bul� samples, representative of the suspcct asbestos-containing building materials
(AC.BM} of eacli houiobenevus ��a (HA), were collected �ccvrdin� lo the �uidelines
published �s Environmcntal Proiection Agency (EPA) Fin�l Rule: Title iT of the Toxic
Substanccs Cantrol Act (TSCA), 15 USC, Seclioas 2641 througli 2654 and in compliance
with 4Q CFR, Part 7tS3. itepresenlative sampling is U�.�ed on the following criteria:
1. The clislribution of the suspect inaterial l.hroufibout the [ IA.
2. The suspect material's physical eharacteristics and applieation.
3. Ranclum samplin� patterns detennined for cach FTA.
Suspect materials sampled and ancilyzed should be c�nsidered to be represcntative of
materials in each HA if:
1. They exhibit sunil�r physical eharaeteristics; and

Bulk sampl�s collected wzre analyzed utiliziug the EPA's �Vtcthod for the Determination of
Asbestos in_Bulk Buildin� Materi�ls (EPA 600/R/116, July, 1993) and the McCmnc Research
Institute'S•The Asbebtos Pariicle Atlas as method.s refereuces.
Analysis of the bulk samples was performcd oa the "date reported," as listed in ihc bulk-
sampl� analysis rcpon.
iV. Notes on Report Format
Suspeet materials alike in appearrsinnce and application were sampled as FIAs_ Suspect
materials were divided into three classiGcarians:
1. Surfacin� z�aatc�a�: sprayed ar troweled onto slructural building member.
2. Thennal systems insulation: any type uf pipc, boiler, tank, or duct insul�ition.
3. Miscellaneous: othcr suspect materials, including floorin�;, ceiling �il�s, insulatinn,
aud fmishing matcrials.
C'ondition assessm�nts were perFormcd by tbe accredited inspector a� the timc of inspection.
Condition assessmentS are listed in the followi.n6 secHon. Ratings of "good," "d�unaged," ancl
"severely dnmaged" are me�uzt to indicate the overall condirinn af the material.
A material in "g��od" condition has nu visible d�ma�c or dcterioration, or showing only very
limited damage or deteriora�ion.
A material in "damaged" condition has the following eharaetex'istics:
Thc surftu:c is crumbling, blistered, water-stained, gouged, marred or otherwise
abraded over less than one-tenlh of ihe surface if the d�nage is evenly
distributed (on�-quarter ix thc damage is localized).
Accumulatipn ofppwder, dust or debris siruil�r in appeatanee tp the suspect n�aterial an

A material in "severely dfunaged" condition has one or �more of the followuig characterisiics:

V. Iuspector Comments
No ACM was identified ciuring ihe bulk sampling conducted at 4220 Spruee W�y in Vail,
Colorado. A total of three (3) bulk sfunples were colleet�d &om the above-referenced
residence. All three bulk s�unples were reportcd as non-dctcct f�r asbest�s (see Appendix A
for labor�tory results of bulk samplcs).

DSC recommends that when ACM is rc:muved/abated, that anly a C:olorado-lic�nscd Asbestns

�Please have the structural engineer verify that the decks have been designed to 100 psf live load.
�Provide a ResCheck report. Change the plans so the r-values on the plans are coinciding with the
