RE: Wilson Residence Permit#B18-0132 1965 Sunburst Drive Vail, CO 81658 *Field Inspection Dear Mr. Pawlak, As requested, structural steel special inspection was conducted per the IBC 2015 Edition- Chapter 17: Structural Tests and Special Inspection and the Town of Vail requirements. I arrived at the job site at the above referenced address at 10:30 AM. I met with Mr. Mark Jones with Evans Chaffee Construction Group to review the material to be inspected and project requirements. Mr. Jones informed me that Custom Structural Steel, Inc. (CSS)of Silt Colorado was the steel fabricator responsible for installing the items of inspection.The welding procedure specifications (WPS)and welder qualification test reports(WQTR)for shop and field welders with CSS were reviewed previously and all were observed to be acceptable with project requirements. Copies of the referenced documents are on file with Mr.Bruce Collins with CSS.The drawings utilized for inspection were structural drawings S1.0, S2.1,and S2.2. Drawing S1.0: Item 8 requires "snug tight"tensioning of all ASTM A325N fasteners. S2.1 was utilized for inspection at the Lower Level of the structure and S2.2 was utilized for inspection of the Ground Floor. The above referenced drawings were prepared by Sipes Architects: Project Number 11108 dated April 11, 2018.Additionally,please refer to the attached E-Mail correspondence from Mr. Michael Strumph,P.E.dated August 15,2018: 10:46 AM accepting the deviation from ultrasonic testing to visual inspection requirements stated in the AISC 360 Code for complete joint penetration(CJP)groove welded connections for the East Wall Moment Frame connection referenced on Sheet S4.0: Detail 4. Visual welding inspection was conducted on all shop and field welds that were visible for inspection.The areas of inspection were at grid locations 7—beyond 8 &E— G.2 and also 5 &C.9—G at the Lower Level. At the Ground Floor level,the areas of inspection were 5 &C.9—G and 1.5 &D.8—F.3. Approximately half of the welding that requires inspection has been obscured from view by installed building materials from other trades. A statement of acceptance by the project engineer of record,Mr. PAGE 2 Michael Strumph,P E. with Monroe and Newell Engineers,Inc.shall be required prior to acceptance by Thunder Mountain Testing. The referenced fillet welds on the items visible for inspection were observed to be in compliance with project specifications and the visual requirements stated in Clause 6 of AWS D1.1 Code: 2015 Edition. High strength bolting inspection was conducted at the above referenced grid locations and floor levels.All bolting was comprised of 3/4"ASTM A325 twist off type tension control bolts each fitted with a heavy hex nut and a hardened flat washer at the bearing face of the nut.At the Lower Level, I observed that one bolted connection lacked evidence of tensioning to project requirements. Mr. Joe Soderstrom with CSS verified tensioning to"snug-tight"conditions under my observation. After the referenced verification,the above referenced bolting was observed to be in compliance with the requirements of the American Institute of Steel Construction(AISC)Research Council on Bolted Connections(RCBC)and project specifications.
