Area #1: Area #3: Area #2: Area #4: Build knee wall at this location to isolate large section above OR from the existing EPDM roof section below that is not experiencing any issues. 03/30/18 OBSERVATIONS OF THE EXISTING ROOF CONDITION: Area #1  Turner Morris was called to investigate a leak below the dumpster staging area at the Vail Valley Medical Center. We were dispatched multiple times for a “reoccurring” leak. Each time we were dispatched we removed more snow and expanded our search area. During our third trip a very large area was removed of snow and over 20 holes were patched. Although we feel that we found most of the holes there is still a very good chance that other holes will leak. Additionally there was a significant amount of water in the roof system, if not dried out the moisture may cause other roof failures later in the roofs life. 03/30/18 Area #2  Turner Morris was called to investigate we found multiple issues with the expansion joint and holes found in the membrane. Although we feel that we found most of the holes there is still a very good chance that other holes will leak. Additionally there was a significant amount of water in the roof system, if not dried out the moisture may cause other roof failures later in the roofs life. 03/30/18 Area #3  Turner Morris was called to investigate a leak above the ambulance garage at Vail Valley Medical Center. We were dispatched multiple times for a “reoccurring” leak. Each time we were dispatched we removed more snow and expanded our search area. It was determined that during the roof installation poor roofing practices were used and the roof is failing around the entire perimeter of the roof. Because the roof doesn’t adhere to the manufacturer’s specifications we recommended a full replacement in this area. 03/30/18 03/30/18 February 13, 2018 PROPOSAL Vail Valley Medical Center Attn:Bill Adochio PO Box 40000 Phone: 970-471-3360 Vail, CO 8165 Email: William.adochio@vailhealth.org PROJECT: Vail Medical Center Roof Replacement 2018 181 West Meadow Drive Vail, CO 81657 SCOPE OF WORK: Membrane Overlay Area #1-4: a. Coordinate with Royal Adhesive representative to complete a pull test of existing EPDM to wood fiberboard to confirm that utilization of low-rise foam can be completed for installation of the new roof system reusing existing insulation. 2. Complete moisture testing of existing perlite insulation below wood fiberboard and replace saturated perlite and wood fiberboard insulation on a time and material basis. This will be completed using a 4” flatstock polyisocyanurate to match height of existing minimum insulation thickness with additional flatstock and tapered polyisocyanurate insulation on top at a cost of $4-$6/sf based on thickness of insulation. 3. Adhere ½ high density Polyisocyanurate substrate board to existing EPDM using a low-rise foam adhesive that is LVOC and useable in low temperatures. 4. Adhere new 60 mil EPDM using LVOC glues that are compatible in temperatures 20 degrees and above. Note that although glues are LVOC they will still emit an odor. 5. Extend EPDM base flashing up walls and rooftop curbs to match height of existing base flashings. 6. Temporarily remove existing cap metal along parapet walls and save for reinstallation. Extend EPDM up and over parapet walls and reinstall existing cap metal. Note that cap metal may appear slightly dented after removal/reinstallation. An option for cap metal replacement can be provided upon request. 7. Tie into existing drains, walls, curbs, roof penetrations per manufacturer’s specifications. Area #1-4 - Warranty: 1.
