This CCD forms a part of the Contract Documents and modifies the original Construction Documents dated July 28, 2014,
as noted below.
Changes to Specification Sections:
1. SECTION 015000-TEMPORARY FACILITIES&CONTROLS: delete "SEE SECTION 01 5213"from sub-section 1.10
FIELD OFFICES
2. SECTION 01 5721-INDOOR AIR QUALITY CONTROLS: Change Sub-Section 1.03 E.to read "Section 23 0593-
Testing,Adjusting& Balancing: Cleaning air ducts,equipment, and terminal units."
3. SECTION 03 5216-LIGHTWEIGHT INSULATION CONCRETE: Fill in blank line in Sub-section 3.01 such that
sentence reads, "Verify openings in existing slab are grouted..."
4. SECTION 03 5400-CAST UNDERLAYMENT: Edit sub-section 2.02 B.: "Place to indicated thickness,with top
surface level to 1/8 inch in 10 ft(1:1000),except at areas intended to slope."
5. SECTION 06 2000-FINISH CARPENTRY: Delete sub-section 1.2 B.
6. SECTION 08 1113-HOLLOW METAL DOORS AND FRAMES: Add sub-section 1.02 D. 13 4905-X-Ray Radiation
Protection.
7. SECTION 08 1416-FLUSH WOOD DOORS:Add sub-section 1.02 D. 13 4905-X-Ray Radiation Protection.
8. SECTION 08 3223-SLIDING/FOLDING GLAZED DOORS/WALLS: Delete blank line in Sub-section 2.03 A. such that
sentence reads, "Glazing: Single glazed, clear, uncoated,fully tempered,..."
9. SECTION 08 7000-DOOR HARDWARE: Revise sub-section 1.01 D. Related documents as follows:
a. Change Item 5.to read, "Division 8 Section "Hollow Metal Doors& Frames."
b. Change Item 6.to read, "Division 13 Section "X-Ray Radiation Protection.""
10. Section 08 7102-DOOR SCHEDULE:
a. Delete "Hardware Set" column from Door Schedule 087102. Door hardware shall be as indicated in
response to submittal.
b. Add two sets of double doors to Door Schedule 087102. See plan changes for more information.
11. SECTION 08 8000-GLAZING:
a. Delete sub-section 1.02, "A. Section 07 2500 Weather Barriers."
b. Revise sub-section 2.01 A.4.To remove blank lines for Total Thickness as windows must match existing.
12. SECTION 09 0561-COMMON WORK RESULTS FOR FLOORING PREPARATION: Revise sub-section 1.02 to add, "B.

a. Show additional area of demolition to correspond with MEP demolition.
b. Add keynote#7 and note, "ACT CEILING AT 8'-0" FIELD VERIFY",to areas of demolition near grid
intersection F/6.
c. Add demolition keynotes#8, 9 and 10 to Demolition Key Notes legend as follows:

d. Add card reader access control devices to west elevator doors.
e. Add Wardrobe cabinets to Patient rooms 209 and 210.
f. Revise Nourish#263, Linen#264, and Soiled Utility#265 to reallocate floor space.
g. Revise column at F/6 to square.
h. Revise floor hatch to indicate areas of lightweight insulating concrete topping and label.

o. Revise Toilet rooms 201A and 202A:
i. increase vanity depth.
ii. change to floor mounted toilets/remove carrier chases.
iii. reconfigure to reduce floor space.
p. Revise dimension at entry to Patient Room 209.
q. Revise Public and Staff toilet rooms#269 and#271A:
i. change to floor mounted toilets/remove carrier chases.
ii. reduce floor space.
r. Reallocate floor space between Staff Locker#271,Cath Lab Storage#272 and Mechanical#273.
s. Add Manifold Room#275.
t. Add room tags to two additional Temp ICU rooms,218 and 219.
u. Revise dimension at new double doors#299 to 46'-9".
v. Add room tag to STORAGE 266(west of SOILED UTILITY 265).
6. A103—ARCHITECTURALTAG, WALL PROT., & EQUIP. PLAN, LEVEL TWO:
a. Change Sheet title to "ARCHITECTURAL WALL PROTECTION &DOOR TAG PLAN, LEVEL TWO".
b. Add missing door tags.
c. Revise Nourish#263, Linen#264, and Soiled Utility#265 to reflect A102 changes.
d. Delete dimensions in Cath Lab and Control. (All dimensions indicated on A102.)
e. Add door tags 280A and 2808 for new doors at elevator lobby.
f. Add door tag for door to new Manifold Room 275.
g. Add corner guards to column at F/6.
h. Add room tag to STORAGE 266 (west of SOILED UTILITY 265).
7. A104—ENLARGED PLANS:
a. See previous plan changes.
b. Remove door tags. (Now shown entirely on sheet A103.)
c. Add interior elevation tag for 3S on A602, Nourish.
d. Relocate interior elevation and equipment and associated tags at Peds/Cath/Exam #220, Nourish#263,
Linen#264, and Soiled Utility#265.
e. Delete tag reading"181"from doorway of PED/OBS#202.
f. Delete 8'-0"dimension from Corridor C295A.
g. Revise Equipment Key to:
i. Add "/shower curtain"to item#14 cubicle curtain.

9. A401—DETAILS:
a. Revise detail#5 to show crown molding.

11. A601—INTERIOR ELEVATIONS:
a. Elevation#3—Control room: Revise dimension to top of countertop to 34". Coordinate with Phillips.
b. Elevation#4—Patient Room 209:
i. Revise note at headwall to read, "Refer to ID drawings for headwall services and locations."
ii. Show Centerline of Patient Bed to match centerline of TV; add dimension to Patient Board and
Clock.
c. Elevation#5—Lockers: Revise location of keynote#3.
d. Elevation#6—Cath Lab: Show low returns at east and west walls per Mechanical.
12. A602—INTERIOR ELEVATIONS:
a. Revise elevations#1, 2 and 3 to match plan revisions.Add south elevation at#3.
b. Delete elevation#7 north wall.
c. Revise elevation#8 to match plan revisions.
d. Revise elevations#9 and 10:
i. Provide heights at item#18 as occurs.
ii. Show Shower grab bars.
iii. Change illustration of toilet tissue dispenser and mirror to match specified items.
