BUILDING PERMIT ISSUANCE TIME FRAME
If this permit requires a Town of Vail Fire Department Approval, Engineer's (Public Works) review and approval,
a Planning Department review or Health Department review, and a review by the Building Department, the
estimated time for a total review will take as long as three (3) weeks.
All commercial (large or small) and all multi-family permits will have to follow the above mentioned maximum
requirements. Residential and small projects should take a lesser amount of time. However, if residential or
smaller projects impact the various above mentioned departments with regard to necessary review, these
projects may also take three (3) weeks to review and approve.
Every attempt will be made by this department to expedite this permit as soon as possible.
I, the undersigned, understand the plan check procedure and time frame. I also understand that if the permit
is not picked up by the expiration date, that I must still pay the plan check fee and that if I fail to do so it may
affect future permits that I apply for.

. PUBLIC WORKS AND THE PUBLIC WAY PERMIT PROCESS
V How it relates to Building Permits:
➢ Fill out the attached check list with the Buildina Permit Application.
If yes was answered to any of the questions then a"Public Way" permit is required. You can pick up an
application at either Community Development, located at 75 South Frontage Road or Public Works, located at
1309 Elkhorn Drive.
➢ Notice sign-offs for utility companies. ALL utilities must field verify (locate) respective utilities prior to signing
application. Some utility companies require up to 48 hours notice to schedule a locate.
➢ A construction traffic control/staging plan must be prepared on a separate sheet of paper. An approved site plan
may also be used. This plan will show locations of all traffic control devices (signs, cones, etc.) and the work
zone, (area of construction, staging, etc.). This plan will expire on November 1St and will need to be resubmitted
for consideration for approval through the winter. Be aware that your resubmission for winter may be denied
depending on the location of construction.
➢ Sketch of work being performed must be submitted indicating dimensions (length, width and depth of work).
This may be drawn on the traffic control plan or a site plan for the job.
➢ Submit completed application to the Public Works office for review. If required, locates will be scheduled for the
Town of Vail electricians and irrigation crew. The locates take place in the morning, but may require up to 48
hours to perform.
➢ The Public Works Construction Inspector will review the application and approve or deny the permit. You will be
contacted as to the status and any requirements that may be needed. Most permits are released 48 hours of
being received, but please allow up to one (1) week to process.
➢ As soon as the permit is approved, the Building Department will be notified, allowing the Building Permit" to be
released. Please do not confuse the °Public Way Permit" with the Building Permit".
➢ NOTE: The above process is for work in a public way ONLY. Public Way Permits are valid only until
November 15th. A new Public Way Permit is required each year if work is not complete. Re-
application each November 15th does not mean an automatic renewal.
I have re d erstand the above.

ASBESTOS ASSESSMENT SURVEY
PRESENTED TO:
Scott Nelson
36 Northwood Ave.
Atlanta, GA 30309
Performed by:
Mr. Brandon Sinkbeil
DS Consultin, Inc.
10988 E. 116t Ave.
Henderson, CO 80640
July 13, 2006
I. Introduction
An asbestos assessment survey was conducted by Mr. Brandon Sinkbeil for GVA Advantis.
Mr. Sinkbeil is the field Representative, EPA accreditatiori #13442. The purpose of the survey
was to locate, sample, and assess suspect asbestos-containing materials (ACM).
II. Structural Design
The structure is a two-level apartment-style home with wood siding, plaster and sheetrock walls,
and both carpet and linoleum floors.
III. Sampling and Analytical Procedures
Random bulk samples, representative of the suspect asbestos-containing building materials
(ACBM) of each homogeneous area (HA), were collected according to the guidelines published
as Environmental Protection Agency (EPA) Final Rule: Title II of the Toxic Substances Control
Act (TSCA), 15 USC, Sections 2641 through 2654 and in compliance with 40 CFR, Part 763.
Representative sampling is based on the following criteria:
1. The distribution of the suspect material throughout the HA.
2. The suspect material's physical characteristics and application.
3. Random sampling patterns determined for each HA.
Suspect materials sampled and analyzed should be considered to be representative of materials in
each HA if:
1. They eibit similar physical characteristics.
2. The application of the sampled material can be correlated to the application of
unsampled material.
Bulk samples collected were analyzed utilizing the EPA's Method for the Determination of
Asbestos in Bulk Building Materials (EPA 600/R/116, July, 1993) and the McCrone Research
Institute's The Asbestos Particle Atlas as methods references.
Analysis of the bulk samples was performed on the "date reported," as listed in the bulk-sample
analysis report. Condition assessments were performed by the accredited inspector at the time of
inspection.
IV. Notes on Report Format
Suspect materials alike in appearance and application were sampled as HAs. Suspect materials
were divided into three classifications:
1. Surfacing material: sprayed or troweled onto structural building member.
2. Thermal systems insulation: any type of pipe, boiler, tank, or insulation.
3. Miscellaneous: other suspect materials, including flooring, ceiling tiles, insulation,
and finishing materials.
HAs found to contain asbestos are described in the following section. Diagrammatic floor plans
showing approximate sample location of these areas are included.
Condition assessments and response-action recommendations for asbestos containing HAs are
also in the following section. Recommendations may be for more than one HA, if materials are
alike.
Recommendations are either "general" or "immediate." Immediate recommendations indicate
imminent hazard and should be addressed as soon as possible.
V. Inspector Comments
The following is a summary of the types of possible asbestos-containing materials found:
Wall plaster and texturing
Sheetrock tape
Linoleum
VI. Asbestos-Containing Homogeneous Area Descriptions and Sample Locations

The linoleum and drywall throughout the unit are all in good condition. The sample taken from
the upstairs hallway closet is representative of all the upstairs walls to be removed, as all appear to
be of the same composition and construction. Implement preventative measures to lessen the
possibility of fiber release due to building maintenance activity until demolition can occur. When
the material is to be disturbed, ensure that only personnel trained in the handling of ACM be
allowed to conduct such activities using appropriate methods (HEPA-filtered vacuuming, wet
cleaning methods, respiratory protection, protective clothing, personnel decontamination, mini-
enclosures, air monitoring, etc.).
