ll�TTRODUCTION:
On May 7th, 2007 an inspection/survey was conducted and 6 bulk
samples were collected from
The Residence .
315 Mill Creek Cr.
Vail, CO. 81657
The purpose of the inspection/survey was to locate and sample
suspected Asbestos containing materials that might be present in the area of
the Residence that is planned for Renovation.
The inspection was made, and the samples were collected by John R
peterman, an A,H.F.Yt.A. and State of Colorado Certifieii Asbestos
Iuspector. Great care was taken during the inspection and sampling to be as
accurate as possible. It should be noted that minimal damage was done to
the existing building strucaues during the inspection so there is no
documentation for unseen conditions or stored items.
All samples were analyzed by DCM Science Lab in Wheatridge, CO.
This laboratory is deemed "Proficient" in the E.P.A.Quality Assurancc (QA)
program for the determination of asbestos in bulk materials, and is
accredited by the American Hygiene Association (AHA).
SAMPLING PROTOCOL:
A random sampling scheme was used to sample the suspect materials that
were discovered if during any future deanolition or renovation worl�
suspect material is discovere,cl that hasn't been sampled and would be
disturbed, work should be halted until the materiai has been tested.

YtENQVATION AREA DESCRTPTION:
The area of the Residence that is planned for selective renovation is cove�ed
in this report. The interior walls are sheetrock with a texture applied to it.
The ceilings are covered in sheetrock Iike the walls. The floors are covered
with stoae and carpe� The exterior walls are covered with stucco.
CONCLUSIONS & RECOMMENDATIONS:
Laboratory analysis, of the bulk samples, indicates that no Asbestos was
detected in any of the bulk samples.

On Thursday, September 27th, 2007, I�nducted a site visit to the Berkowitz r+emodel at the request of the coMractor,
Gary Case. The tollowin9 items w�er'e brou9ht to myattention:
1. At the roof iraming for the new domier over the guest bedroom, H�ere are a few rafters on the south side of
which bear on a flat 2x6 running abng the new valley. Blodcing has been added beneath these rafters to incxease the
bearing area.
2. At the top of the doRner over the guest bedroom, there is a short rafter (approximately T-0 long) which is
notched to bear on tl�e existing steel beam.
3. 2x10 blodcing was not installed on the garage roof undem�lh a wall above, as indichted on plan- Gary
informed me that blodcing would be ir�stalled as per plan.
4. A beam speafied as (3r1 3/4x14' LVL on plan was aoaderrtally replaced with a(3}1 3/4x11 7/8" LVL. The
oorrec� beam had been instatled at the time of my site visit. The top of this beam was notd�ed to be�ar on the flange of
the ebsting steel beam.

The ESL 449/448 Series self�iiagnostic, four wire
smoke detectors continually monitor their own
sensitivity and operational status, and provide a
visual trouble indication if they drift out of sensitivity
range or fail internal diagnostics. This unique,
patented technology meets NFPA 72 field sensitivity
testing requirements without the need for external
meters.
Additional diagnostic information is activated by
applying a magnet near the detector's integral reed
switch. This initiates a self-diagnostic routine and
provides visual indication of sensitivity level, or if
service is required.
This series is easily deaned by simply replacing
ESL's proprietary field-replaceable optical chamber. All
models are designed to reduce false alarms from
dust, insects, RFI, and external light.
An integral combination rate-of-rise and fixed
135°F (57°C), 50-foot rated, heat sensor is available
with all "T' model detectors (see selection guide),
allowing latching of the alarm for either smoke or
heat. The 449CSRH includes an isolated alarm
output for heat and activates an internal non-latching
sounder (local alarm) for smoke, making it ideal for
motel/hotel and dormitory rooms whe�e smoking is
permitted.

Archibectural and Engineering Specifications
The ESL 449 Series bw�'ofile, self�iiagnostic, four-wire
smoke detectors +nrork on the light scattering principle. A
pulsed infrared light-emitting diode serves as the light source,
and a high-speed photodiode as the sensing element. When
the amount of light �efleded onbo the photodiode reaches ihe
sensitivity setting, the smoke sampling rate increases. Three
(3) successive smoke sensings above the sensitivity setting
are required to sound an alarm. This design has superior
protection against false alarms ca�sed by dust, insects, RF
and ambient-light.
A confirmed alarrn causes the normally flashing power
indicator LED to light continuously and the alarm relay to
operate. A trouble indication is automatically displayed by
flashing the LED every second. This meets NFPA 72 field
sensitiviry testing requirements. An internal 85 d6 hom
{available in "S' models) emits a temporal 3 when the
detector alarms and a steady tone when power polariry is
reversed.

During the walkthrough, the following items were brought to my attention:
Roof Framing (sheet S5) -
1. A portion of U�e new roof framing located over the storage/bathroom adj�ent to the office was framed with
2x8's ripped to a depth of 6 3/4". These 2x8's span approbmatey 6'-0, and are supported by 1 3/4 x11 7/8"
LVL beams.
Upper FloodLow Roof Framing (sheet S4) -
1. A(2}1 3/4x91/2" LVL beam was added to support an existing (3)1 314x11 7/8 LVL beam which supports the
stair landing at the existing offce.
2. 1 3/4x11 7J8' LVL's were used to frame the famdy room floor instead of 2x12's as indic�ted on plan.
3. (4}Zx6's w�e installed under each side of the W10x19 steel beam.
4, �c1 p's were used to frame a small portion of the floor near the garage and irorrt ermy. The 2x10 floor joists
span appropmatey T-0.
Main Floor Framing (sheet S3) —
1. The tioor over the spa area was framed flush with the adjaoent epsting floor, instead of 5" belaw as indicated
on plan.
2. 2x10's were used to irame the floor of the new sitting room, instead of 2x12's as indicated on plan. These joists
span appro�amate{y 8'-0, with a 3'-0 cantilever.
3. LVL floor joists for the new dedc adjacerrt to the biNiard room have been ripped to a minimum depth of 7 7/4',
instead of 91/4" as indicated on plan.
4. Floor joists for the b�liard room hang off of an 11 7/8' LVL ledger, instead of bearing diredly on the wa��•
Aooeptable jaist hangers have been provided• pmately 8'-6'.
