The two previous visits were on 12/23/2003 and 1/7/2004. The floor slabs were placed during the late summer of 2003. There are in-floor heat tubes in the slabs, and the heat was turned on in November. Though I did not observe it at the time, I have been told that the slabs were placed on a 4" layer of gravel and that there is welded wire fabric in the slabs, according to the original structural engineer's recommendations. The cracks in the slabs were only recently observed, but they may have actually appeared earlier and not been noticed because of construction materials being stored there. I observed fairly numerous random cracking throughout the lower level slabs. There were some saw-cut control joints, but they were not located appropriately to prevent shrinkage cracks from occurring. In the southwest bedroom, the floor slab appears to have an upward heave in the center of the room. There are cracks in the concrete in all directions from the center of the room, and a carpenter's level indicated that the slab slopes approximately 1/4" in 4' away from the center. The slab levels out toward the perimeter walls, and is level along the perimeter walls. The northwest bedroom and media room floors show similar characteristics, but to a lesser extent. The southeast bedroom has a saw-cut control joint running north-south across the room, and the slab is level to the west side of the control joint, and slopes downward approximately 1/2" in 4' to the east side of the control joint. The exterior patio slab outside the lower level along the west side of the residence, which has a stamped pattern surface, has a shrinkage crack parallel to and approximately 6' away from the foundation. There are no control joints in this exterior slab. There are no signs of drywall cracking or other indications of foundation movement. The framed floors at the floor above are level. It appears as though the cracks in the slabs are shrinkage cracks resulting from the lack of adequately placed control joints. The apparent heaving in the centers of the rooms is difficult to explain, because the primary causes for a slab to heave after it has been placed are expansive clay soils or frozen subgrade, neither of which conditions exist here. I suspect that the slabs were originally placed out of level, and when the shrinkage cracks occurred, they magnified the problem. In the case of the exterior patio slab, there could be some minor settlement toward the downhill side of the site, combined with the lack of control joints, causing the crack parallel to the foundation. Box 7431 Avon, CO 91620-7431 Re: Lot 35 Dear Greg: The following items were noted in our 9/17/03 visit to the site as either requiring correction or completion: Item #l: The micro-lam lean in the Garage in the West Unit needs to have 3-2x6 stud packs installed at each end under the beam bearing points. Item $12: Continuous mid-height 2x4 blocking needs to be installed between each stud in the 2x4 bearing fuming walls in the Media Room of the East Unit. Item iii: The exiting steel beam at the eastern edge of the loft in the East Unit needs to be reinforced to accommodate the 'removal of the existing stair wall, see the attached plan. This existing beam needs to have the lag bolts called for on the plans installed at each end connecting it to the bearing stud packs. Item #4: The construction of the new exterior deck on the second.floor of the East Unit will require the installation of new pressure treated 2x10 joists for the deck floor structure, See the attached partial plan. Item #5: In the West Unit, the new north-south micro-lam roof beam crosses directly above the existing east-west Glu-Lem beam. A full width plywood shim should be installed to allow the micro-lam to bear on top of the Glu-Lam. Item #6: An additional fireplace is to be installed along the cast wall of the Great Room of the West-Unit. The flue pipe will exit through the exterior wall and interrupt the stud pack 'that is supporting the existing roof Glu-Lam. This stud pack should be supported by a new 3-1 '/•' x 9 1/s" mirco-tarn header which will be supported by 3-20 stud pack at each end extending to the foundation. Item #7: Joist hanger, beam hanger and I13 clip installation is in progress in the West Unit but was not complete at the time of the visit. Item #8: In the West Unit, Bedroom #3. above the shower door, install a 2-1 W x 9 %z" micro- lam beam to support the floor joists of the floor above. Bear the micro-lam on 3-20 stud packs at each end.
