Construction Activities Observed: The contractor was performing snow removal.
Equipment in Use:
H-P/Kumar's Site Activities: As scheduled, we visited the site to perform an epoxy dowel
inspection. The contractor was not ready for an inspection.
Verbal Communication: Jim Meehan with Mountain Home Construction requested we return
the following day 11-6-18.

Area of Inspection: Epoxied Dowels into existing Foundation wall.
Reference Documents Structural Drawings provided by the contractor.
Used:
Inspection Notes: Dowel holes were correct diameter and met minimum depth required and
were blown free of debris prior to application of HILTI HIT-RE 500 V3 as
per manufactures specifications.
Inspection Status: ❑ Preliminary ® Final
® Concrete reinforcement as noted above was judged to be in reasonable conformance with
the referenced construction documents.
