Included in the folderto addressthe outstanding items requested by the town at the meeting are: 1.) An amended letter from the structural engineer specifically affirming the occupancy of the garage may be maintained duringthe removal of soil from the roof. 2.) An exhibit illustrating the design/details for the covered walkway from the egress dooron the NW corner to the ROW. Based on our discussions I believe the only other outstanding items raised by the Town staff are as follows: 1.) Assurances that the parking garage willnot remain an eyesore: As noted,we are prepared to provide a cash surety as previously discussed and are amenable to conditioning the approval with a date by which the remedial work must be started by should the vertical construction not commence priorto. 2.) CDOTPermit: As discussed,the only remaining item that is holding up the CDOT permit is the issuance of an insurance certificate. We understand that Tom Kassmel is willing to condition the start of the work upon the CDOT permit being issued. 3.) Fire Sprinkler System Test: We have been actively working to clarify the requirements of the test and schedule the it with Mike Vaughn and understand that this is a condition of our approval to proceed with the work. Project Description a) The information provided in this document and in the attached referenced documents are pertinent to the demolition and due diligence necessary to verify existing conditions in preparation f orthe construction of a new residential building. b) The building is to be constructed on top of an existing concrete parking structure (currently covered in vegetation), located at 434 South Frontage Rd East,Vail, Colorado 81657 [Attachment A-Vicinity Map]. c) Given the scope and limited site area, most of the site area beyond the building footprint will be disturbed. Soil disturbance will be limited to excavation of utilities, and removal of soil and vegetation on top of the existing parking garage. 2. Project Communication a) A notice has been distributed to adjacent property owners HOA including an invitation to meet them foran informal preconstruction meeting. b) Throughout the course of construction, Shaw will distribute to the neighboring building HOA's, monthly updates on project to keep them appraised of the work completed and forthcoming. c) A project sign will be posted at the entrances to the site on South Frontage Road [Attachment B — Project Sign]. Specific warning and safety signage will also be posted where appropriate given the potential hazards. 3. Pedestrian Protection a) Shaw Builders will oversee all construction related work and will uphold utmost respect to public safety and welfare. b) The construction project will comply with pedestrian safety per TOV requirements,the Americans with Disability Act,and applicable IBC Building Codes in areas outside the construction zone, where the public access is maintained. c) Existing egress paths and associated life safety systems (fire alarm, fire sprinklerand emergency lighting) in the parking garage will remain functional during demolition. The existing fire hydrant(s)and FDC connections will also remain functional during the demolition. d) Thoughtful due care will be made to ensure safe passage of the Vail Mountain View residents into and out of the parking garage, including emergency exits,should an emergency event arise. To protect the emergency egress route out of the northwesternmost exit, a covered walkway will be installed extending from the exit door to the public way (frontage road sidewalk)to the north. The covered walkway will be constructed f orits intended use with adequate lighting. 4. Project Implementation a) The project is scheduled to start work as soon as the permit is issued, and crews are able to mobilize. b) Applicable permits (demolition, ROW and CDOT Access) will be obtained before the commencement of the work. c) Construction work hours will be in accordance with the most current Town of Vail (TOV) Construction Handout (7:00 am to 7:00 pm Monday through Saturday), no work outside the site afterdark(variable depending on date and weather conditions). In addition, specific requirements/restrictions on construction activities will be adhered to, including during TOV Special Holidays and the TOV Special Events. Any work planned during these days shall only be permitted with advanced approval from the TOV. d) A Staging Plan [Attachment C] has been created to communicate the site logistics during the demolition stage. In preparation of these plans, detailed thought was put into the public's Vail Mountain View Phase II CONSTRUCTION MANAGEMENT PLAN safety in consideration with the anticipated activities. Should amendments to these plans be warranted, updated versions will be prepared and communicated. e) To secure the site along the frontage road (adjacent to the property line) concrete jersey barriers will be placed and topped with a 3' high chain-link fence that will be covered with a green mesh wind screening, as required by the TOV. The remaining perimeter of the project site will have a 6' high temporary chain-link fence with the same green screening applied. In addition, on top of the parking structure, safety barriers will be installed to protect both the workers from falls as well as the Phase I residents from debris. If any marketing screening is desired the plans and permit application will be submitted forTOV approval priorto installation. 5. Material Loading a) Materials removed from the site will be loaded in the Loading Zone, as indicated on the Staging Plan. An exhibit was created to demonstrate the gate locations and zone are appropriate to accommodate entering and exiting the zone. b) A Truck Route Plan [Attachment D—Truck Route]was created to communicate to all contracted trucks the approved routes to and from the project site to avoid disruptions. c) It is anticipated that the heaviest trucking/ hauling will occur during this stage. Approximately 1,100 CY of existing soils will be removed from the site utilizing end dumps (15—17 CY), tandems (20—25 CY)or a combination of both types of dump trucks, which equates to approximately 55—75 loads. f) Special attention will be paid to ensure the roadways will be kept free of dirt and debris. 6. Traffic Control and Parking a) Shaw Builders will utilize the following parking solutions f orthe construction workeraccess to the site as part of the parking plan: (1) Parking will be in designated spots within Phase II parking garage f orlimited authorized project team members only (Ownership, Shaw Supervisors &Consultants, and pre-designated subcontractorsupervisors). (2) All other construction personnel will be required to carpool utilizing vans which may park at the Village Parking Structure located at 241 E Meadow Drive. Workers may park at the designated street parking along the N. Frontage Road south of Vail Fire Dept Station 3 where vans will transport them to/from the site at designated times [Attachment E—Shuttle Route]. Other pick-up/ drop-of fpoints may be utilized by subcontractors at other locations outside the TOV. b) This document will be incorporated into all Subcontract/Supplier contracts to ensure that all workers and delivery drivers are in strict compliance with the parking and carpooling plans. Any new workers will be similarly informed as part of preconstruction meeting/project orientation process. c) Traffic control as required f ortrucking, heavy equipment operations, material, deliveries and crane movements will be fully compliant with the TOV and CDOT requirements. Certified flaggers will be onsite during heavy equipment operations,as required,to safely direct traffic (pedestrian and vehicular) on the Frontage Road into and out of the designated project access points. In the interest of public safety these operations will not be allowed during inclement weather,or of terdark. 7. Emergency Vehicle and Personnel Access a) Shaw Builders will maintain emergency vehicle access, on and around the site, including but not limited to police,fire, and ambulance services. A clear area forfire truck and other emergency vehicle staging will be continuously maintained as indicated on Staging Plan Vail Mountain View Phase II CONSTRUCTION MANAGEMENT PLAN [Attachment C—Staging Plan], in addition access to the path to Phase I on the east side of the site. b) Priorto the start of demolition, a Site-Specific Safety and Emergency Response Plan will be created by Shaw Construction and will be strictly enforced to during construction. All subcontractors will be contractually bound to the plan and it will be reviewed with all workers prior to starting work on the site as part of Shaw's mandatory project orientation. c) Movements of any kind(trucking, heavy equipment, etc.)will be halted immediately in the event of an emergency,to enable safe access into the site by emergency vehicles and personnel as well as safe egress of the occupants of fsite. 8. Dust Control a) When exporting dirt and debris,dump trucks will utilize dirt screens to keep dirt& dust from being blown out onto the streets. Gravel tire cleaners will be located at the exits of the construction site to prevent mud from being tracked on the streets. b) A street sweeper will also be used to clean the streets. c) Airborne dust will be managed during the work by watering either by hand or with equipment specifically designed forthis purpose. 9. Snow Containment and Removal a) Snow will be removed from the project work areas, vehicle& pedestrian access areas, and piled at designated locations to be loaded into dump trucks and removed from the project site. b) Snow will not be piled onsite f orextended periods of time due to the constraints of the project site. The purpose of our review was to determine to acceptability of using a skid steer loader on the roof to remove the existing soil. For our analysis we looked at the CAT 299D2 track mounted skid steer with a total weight of approximately 11,000 lbs. and a load capacity of 4,600 lbs. (approximately 1.7 cubic yard). The existing roof is capable of supporting the skid steer provided it is working on the concrete surface into the existing soil. It is not capable of supporting the skid steer running on top of the existing soil. Provided the conditions above are complied with, there is nothing that prevents the parking structure below from remaining occupied during the soil removal.
