Asbestos Testing, Project Design, and Consulting
INTRODUCTION:
On January 16`h, 2009 an inspection/survey was conducted and 3 bulk
samples were collected from the:
The Residential Building
1763 Alpine Drive
Vail, CO.
The purpose of the inspection/survey was to locate and sample
suspected Asbestos containing materials that might be present in the area of
the Residence that is planned for fire damage renovation.
The inspection was made, and the samples were collected by John R.
Peterman, an A.H.E.R.A. and State of Colorado Certified Asbestos
Inspector. Great care was taken during the inspection and sampling to be as
accurate as possible. It should be noted that minimal damage was done to
the existing building structures during the inspection so there is no
documentation for unseen conditions or stored items.
All samples were analyzed by DCM Science Lab in Wheatridge, CO.
This laboratory is deemed "Proficient" in the E.P.A.Quality Assurance (QA)
program for the determination of asbestos in bulk materials, and is
accredited by the American Hygiene Association (AHA).
SAMPLING PROTOCOL:
A random sampling scheme was used to sample the suspect materials that
were discovered. If during any future demolition or renovation work,
suspect material is discovered that hasn't been sampled and would be
disturbed, work should be halted until the material has been tested.

Re: Gury Residence, fire alarm system replacement
This letter is in regards to the fire alarm system retrofit forthe Gury Residence, located at 1763
Alpine Drive, Vail, Colorado. This existing home will undergo afire alarm system replacement
completed by Thul Electronics.
Our scope of work includes the installation of a new fire alarm panel (Ademco Vista 50P) and
new fire alarm devices throughout the house. Existing locations will be reused.. As there are no
drawings available, this letter is a request to allow the fire alarm contractor, Thul Electronics, to
proceed with the understanding that a fire protection engineer will review the device locations to
ensurecompliance.
We will ensure that the following devices are provided in the indicated locations:
• Smoke detectors in all sleeping rooms, located centrally in the room, accommodating
ceiling features including beams, slopes and peaks.
• Smoke detectors in areas directly outside of sleeping rooms, at the top and bottom of
stairs, ensuring at least one detector per floor is provided.
• Heat detectors in Laundry Rooms, the Kitchen and the Garage.
• Horns as needed to achieve 75dB at the pillow of each sleeping room.
We understand that all device locations are subjeetto approval by Vail Fire & Emergency
Services. I will provide an as-built battery calculation and zone list at the completion of the
project.
