(305 mm) OC. When optional spray-applied fire resistive material is used on the steel deck, ceiling runner secured throughഀ
spray-applied material to valley of steel deck with min 3/16 in (5 mm) diam steel masonry anchors spaced max 12 in. (305 mm) OC.ഀ

On August 18`h, 2008 an inspection/survey was conducted and 5 bulkഀ
samples were collected from:ഀ
Golden Peak Lodgeഀ
458 Vail Valley Driveഀ
Unit #1ഀ
Vail, CO. 81657ഀ
The purpose of the inspection/survey was to locate and sampleഀ
suspected Asbestos containing materials that might be present in the area ofഀ
the Residence that is planned for Renovation.ഀ
The inspection was made, and the samples were collected by John R.ഀ
Peterman, an A.H.E.R.A. and State of Colorado Certified Asbestosഀ
Inspector. Great care was taken during the inspection and sampling to be asഀ
accurate as possible. It should be noted that minimal damage was done toഀ
the existing building structures during the inspection so there is noഀ
documentation for unseen conditions or stored items.ഀ
All samples were analyzed by DCM Science Lab in Wheatridge, CO.ഀ
This laboratory is deemed "Proficient" in the E.P.A.Quality Assurance (QA)ഀ
program for the determination of asbestos in bulk materials, and isഀ
accredited by the American Hygiene Association (AHA).ഀ
SAMPLING PROTOCOL:ഀ
A random sampling scheme was used to sample the suspect materials thatഀ
were designated. If during any future demolition or renovation work,ഀ
suspect material is discovered that hasn't been sampled and would beഀ
disturbed, work should be halted until the material has been tested.ഀ

The interior walls are covered in textured sheetrock, and the ceilings areഀ

carpet, and ceramic floor tiles.ഀ

Laboratory analysis of the composite sheetrock samples indicate that noഀ

1. Parkin- All parking will take place in the Vail public parking garages. GSCCഀ
will also provide subcontractor employees shuttles to the job site.ഀ
2. Restrooms- All workers will be required to use the designated restroom in unit 1.ഀ
3. Trash Removal- Propose placing a trash truck on the north side of the garageഀ
doors at a two hour intermittent time line during demolition (September). Allഀ
demolition debris will be placed in the trash truck through the unit 1 bedroomഀ
window. Once demolition is completed trash will be removed via pick-up trucksഀ
loaded on the north side of the buildings garage.ഀ
4. Hot Tub Removal & Installation- GSCC will coordinate/schedule theഀ
removal/installation of the existing/new hot tub via crane the hot tub company.ഀ
Town of Vail. Right of Way permits will be obtained and flaggers will be used forഀ
the removal/installation.ഀ
5. Flow of Materials- Demolition debris and daily trash removal will be taken fromഀ
the unit in trash receptacles down the stairs to the GPR lobby/garage level andഀ
loaded into dumpster/pick-up trucks. Construction materials will be unloaded intoഀ
the GPR garage and taken up the stairs as necessary. Larger items which will notഀ
fit through the stairway door will be taken up the elevator and will be completelyഀ
wrapped for protection.ഀ
6. Deliveries- GSCC will coordinate/schedule deliveries with the GPR. Flaggers willഀ
be used for deliveries as necessary.ഀ
7. Worker Access- Worker access will be via the entry gate & stairwell to the thirdഀ
level to the unit. Workers will be required to wear booties when leaving the unitഀ
to keep dust levels down in the hallways. (see photo)ഀ
8. Storage of Materials- GSCC will use 2 of the GPR Unit 1 garage parking spacesഀ
enclosed with a lockable screened chain link fence for storage of materials duringഀ
construction (see photo).ഀ
