Dear Mr. Counter,
At your request, Foothills Environmental Inc. (FEI) conducted a limited asbestos inspection
of suspect materials that may be impacted during a proposed renovation at The Lodge at Vail
condominium units 332 and 334. Mr. Lyle Ardourel, an asbestos inspector certified by the
Colorado Department of Public Health & Environment conducted the limited asbestos
inspection. Reservoirs Environmental, an independent laboratory accredited by the National
Voluntary Accreditation Program (NVLAP) and the American Industrial Hygiene
Association (AIHA) analyzed the samples utilizing Polarized Light Microscopy (PLM). '
The bulk sampling of suspect building materials was conducted to identify any Asbestos
Containing Material (ACM) in the Units before remodeling activities begin. The
Environmental Protection Agency (EPA) defrnes an ACM as a material with an asbestos
content of greater than one percent (l%)by area. Both friable and non-friable materials were
sampled. A friable material is a material that when dry may be crumbled, pulverized, or
reduced to powder by hand pressure. Because friable matsrials are more easily damaged and
more likely to release fibers into the air, they are of greater concem than non-friable ACM.
Random bulk samples of suspect building materials were collected according to the
guidelines published as the Environmental Protection Agency's Final Rule: Title II of the
Toxic Substances Control Act (TSCA), 15 USC, Sections 2641 through 2654 and in
compliance with 40 CFR, Part 763 and the Colorado Department of Public Health and
Environment (CDPHE) Regulation #8.
All accessible interior areas were inspected for ACM. FEI did not inspect outside of the
proposed scope ofwork, underground conduit, electrical panels instruments or other
appurtenances.

The Lodge at Vail is located at 774 Gore Creek Dr. in Vail, Colorado. The Lodge is a three-
story structure on a concrete foundation. The exact date of construction is unknown but is
estimatedto be the late 1960s to early 1970s.

Removal, in accordance with the Colorado Department of Health and Environment's
Regulation No. 8, is required if the amount of asbestos-containing material that is friable or
will be made friable during renovation or demolition activities exceeds the trigger level of
160 square feet. Requirements set forth in the Occupational Safety and Health
Administration's Construction Asbestos Standard. 29 CFR 1926.110l must also be followed
when handling asbestos containing material
Texturing Cornpounds are classified as surfacing materials and must be removed prior to
building demolition or remodeling. The Asbestos Containing texturing compounds are
associated with all the Drywall throughout both Units 332 and 334. The ACM texturing
compounds are located on the walls and ceilings throughout both Units with the excepion of
the Kitchen Soffit in Unlt#332. The original Kitchen ceiling above the Kitchen Soffit in Unit
#332 is presumed to be ACM until sample analysis proves otherwise.

This report describes the locations and ponditions of ACM identified in the facility during the
inspection. FEI represents that our services are performed within the limits prescribed by
applicable regulations and in a manner consistent with the level of care and skill ordinarily
exercised by other professional consultants under similar circumstances. No other
representation is made to the client, expressed or implied, and no warranty or guarantee is
included or intended.

BUILDING PERMIT ISSUANCE TIME FRAME
If this permit requires a Town of Vail Fire Depaftment Approval, Enginee/s (Public Works) review and approval,
a Planning Depaftment review or Health Department review, and a review by the Building Depaftment the
estimated time for a total review will take as long as three (3) weeks.
All commercial (large or small) and all multi-fami[ permits will have to follow the above mentioned maximum
requirements. Residential and small projects should take a lesser amount of time. However, if residential or
smaller projects impact the various above mentioned departments with regard to necessary review, these
projects may also take three (3) weeks to review and approve.
Every attempt will be made by this department to expedite this permit as soon as possible.

Unlawful deposits: Subject to subsection C thereof, it is unlawful for any person to lifter, track or deposit, or cause to
be liftered, tracked or deposited, sand, gravel, rocks, mud, dirt, snow, ice, or any other debris or material upon any
street, sidewalk, alley or public place, or any portion thereof,

Notice; Abatement: The Director of Public Work may notifo and require any person who violates or causes another
to violate the provision of subsection A hereof, or who has in the Directo/s employment a person who violates or
causes another to violate the same, top remove such sand, gravel, rocks, mud, dirt, snow, ice or any other debris or
material within twenty four (24) hours after receipt of said notice by the Director of Public Works. In the event the
person so notified does not comply with the notice within the period of time herein specified, the Director of Public
Works, or other authorized agent, may ciluse any such sand, gravel, rocks, mud, dirt, snow, ice, debris or any other
material to be removed from any street or alley at the expense of the notified.
Summons and Penalty: As an alternative to the notice for removal provided in subsection B above, any person
who violates or causes another to violate the same, may be issued a summons to appear before the Municipal Court of
the Town for said violations, and upon being found guilty of a violation hereunder be punished as provided in Section
1-4-1 of this code.

such conditions as to interfere with the free movement of vehicular traffic or proper street or highway maintenance.

Whenever any police officer finds a vehicle attended or unattended, standing upon any portion of a street or upon any
place within this Municipality in such a manner as to constitute a violation of any section of this Afticle, or left
unattended for a period of twenty four (24) hours or more and presumed to be abandoned under the conditions
prescribed by Colorado Revised Statutes section 42-4-1t02, as amended, the officer shall require the vehicle to be
removed or @use it to be removed and placed in storage in the nearest garage or other place of safety designated or
maintained by this Municipality, and the charges for towing and storage of such vehicle shall be charged to the owner

Introduction:
On Aprit 22,,2006rBoulder Environmental Management, Inc.'s air monitoring specialist
Mark Symmank, collected final air clearance samples from 174 Gore Creek Drive'
VaiI, Colorado
Prior to testing, a visual inspection was performed to confirm the completeness of
asbestos removal and cleanup. The visual inspection was perfomed in accordance with
the American Standard for Tlsting and Materials' "standard Practice for Visual
Inspection of Asbestos Abatement Projects". All abated asbestos materials, including
contaminated dust, debris, or residues were satisfactorily determined to have been
removed.
Air samples were then collected inside the contained area utilizing aggressive air
sampling techniques as described in 40C.F.R. Parr763,Appendix A to Subpart E (EPA
199s).
Five air samples and two blanks were analyzed utilizing Phase Contrast Microscopy
(PCM) emplbying the NIOSH 7400 method. The results of the PCM analyses as follows:

Environmental Management, Inc. Boulder Environmental Management, Inc. successfu lly
participates in the American Industrial Hygiene Association's Proficiency Analytical
testing Prograrn (PAT) for determining concentrations of airbome asbestos frbers.

Sounder bases combine an audible notification applialrce
and a TrueAlarm analog sensor to provide:
Digital Gommunication of Analog Senslng.
Sensors provide an analog measurement that is digitally
communicated to the conlrol panel where it is analyzed
and an average value is determined and stored. An alarm
or other abnormal condition is determined by comparing
the sensor's present value against its average value.
Intelligent Data Evaluation. Monitoring each sensor's
average value provides a software filtering averaging
process that compensates for environmental facton (dust,
dirt, etc.) and component aging, providing an accurate
reference for evaluating new activity. The result is a
significant reduction in the probability of false or
nuisance alarms caused by shifts in sensitivity, either up
or down.
Gontrol Panel Selection. Peak activity per sensor is
stored to assist in evaluating specific locations. The alarm
set point for each TrueAlarm sensor is deterrnined at the
control panel, selectable as more or less scnsitive as the
individual application requires.
Timed/Multi-Stage Selection. Alarm set points can be
programmed for timed automatic sensitivity selection
(such as more sensitive at night, less sensitive during
day). Control panel programming can also provide
multi-stage operation per s€nsor. For example, a 0.2ulo
level may cause a waming to prompt investigation while a
2.5Y:o level may initiate an alarm.
Sensor Alarm and Trouble LED lndication. Each
sensor base's LED pulses to indicate communications
with the panel. Ifthe control panel determines that a
sensor is in alarm, or that it is dirty or has some other t]?e
of trouble, the details are annunciated at the control panel
and that sensor base's LED will be turned on steadily.
During a system alarm, the control panel will control the
LEDs such that an LED indicating a trouble will reh.rrn to
pulsing to help identify the alarmed sensors.

Base mounted address selection allows the address
to remain with its programmed location when the sensor
is removed for service or type change. Access is from the
front under the removable sensor.
Automatlc sensor type identification provides
default sensitivity when substituting sensor types.
Differenl sensor types can be easily interchanged to meet
specific location requirements. This feature aiso allows
intentional sensor substitution during building
construction. When conditions are temporarily dusty,
instead of covering the smoke sensors (causing them to be
disabled), heat sensors may be installed without
reprogramming the control panel.
Integral red LED indicates power-on by pulsing, or
alarm or trouble when steady on. The exact status is
annunciated at the fire alarm control panel.

Simplexo 4902 Series speakers provide high quality sound
for emergency fire alarm use as well as for background
music. The moisture-repellent speaker is designed for

The multi-tapped speakcr transformer accommodates

Amendment to the 2002 N.E.C, Town of Vail Ordinance 4, Series of 2005
Overhead seruices are not allowed in the Town of Vail.
Underground services shall be in conduit (PVC) from the utility ffansformer to the electic meter, main
disconnect switch, and to the flrst electrical distribution circuit breaker panel.
The main disconnect switch shall fu readily accessible, andlocated next to the meter on the exterior wall of
the structure. All underground conduits are required to be inspected before back-filling the trench.
In multFfamily dwelling units, no electrical wiring or feeder cables shall pass from one unit to another. Common
waffs and spaces'are exempt,
NM Cable (Romex) an be used only in single and multi-family dwellings net exceeding 3 $ortes,
Type NM cannot be used in any building mixd with Type A,B,E EUI,tl &S occupanci*,
Aluminum anductorc smaller than size #8 are not permitted with the Town of Vail.
" TOWN OF VAIL ELECTRICAL PERMIT GUIDEUNES
All installations of e)terior hot tubs or spa! require a DRB approval from planning. This application will
not be accepted without a copy of the DRB approval form attached (if applicable).
If this permit is for installation of an exterior hot tub or spa on a new elevated platform or deck over
30" above grade, you must also obtain a building permit.
If this permit is for installation of an exterior hot tub or spa on any existing deck or elevated platform,
a structural engineer must review the existing condition and veriry that it will suppott the added
concentrated load. Please provide a copy of the structural engineers wet stamped letter or drawing
with this application.
If this is a remodel in a multi-family building with a homeowners association, a letter of permission
from the association is required.
If this permit is for a commercial space, two (2) sets of stamped drawings are required. Electrical
one4ine and panel schedules are reguined if load is added or distribution is alhrcd.

General
Description
The Series LFll (TY2596) Residential
Concealed Pendent Sorinklers are
decorative, fast response, fusible sol-
der sprinklers designed for use in resF
dential occupancies such as homes,
apartments, dormitories, and hotels.
The cover plate assembly concealsthe sprinkler operaling components
above the ceiling. The flat profile ofthe
cover plate provides the optimum aes-
thetically appealing sprinkler design.In addition, the concealed design of
the Series LFll (TY2596) provides 1/2inch (12,7 mm) vertical adjustment.
This adjustment reduces the accuracyto which the fixed pipe drops to the
sprinklers must be cut 1o helo assure
a perfect fit installation.
The Series LFll are to be used in wet
pipe residential sprinkler systems for
one- and two-family dwellings and mo-
bile homes per NFPA 13D; wet pipe
residential sprinkler systems for resi-
dential occupancies up to and includ-
ing four stories in height per NFPA
13R; or, wet pipe sprinkler systems for
the residential portions of any occu-
pancy per NFPA 13.
The Series LFll (TY2596) has a 4.2
(60,5) K-factor that provides the re-
quired residential flow rates at reduced

The Series LFll (TY2596) Residential
Concealed Pendent Sprinklers are
shipped with a Disposable Protective
Cap. The Protective Cap is temporarily
romoved for installation, and then it
can be replaced to help protect the
sprinkler while the ceiling is being in-
stalled or finished. The tip of the Pro-
tective Cao can also be used to mark
the center of the ceiling hole inlo plas-
ter board, ceiling tiles, etc. by gently
pushing the ceiling product against the
Prolective Cap. When the ceiling in-
siallation is complete the Protective
Cap is removed ind the Cover Plate
Assembly installed.
WARNINGS
The Series LFII (TY2596) Residential
Concealed Pendent Sprinklers de-
scribed herein must be installed and
maintained in compliance with this
document, as well as with the applica-
ble standards of the National Fire Pro-
tection Association, in addition to the
standards of any other authorities hav-
ing jurisdiction. Failure to do so may
impair the pertormance of these de-
vices.
The owner is responsible for maintain-
ing their fire protection system and de-
vices in proper operating condition.
The installing contractor or sprinkler
manufacturet should be contacted
with any questions.

the scope of the provided citeria, referto the Residential Sprinkler Design
Guide TFP490 for the manufacturer's
recommendations that may be accept-
able to the Authority Having Juisdic-
tion.
System Type. Only wet pipe systems
may be utilized.
Hydraulic Design, The minimum re-quired sprinkler flow rate for systems
designed to NFPA 13D or NFPA 13R
are given in Table A as a function of
lemperature rating and lhe maximum
allowable coverage areas. The sprin-
kler flow rate is ihe minimum reouired
discharge from each of the total
number of "design sprinklers" as
specified in NFPA 13D or NFPA 13R.
For systems designed to NFPA 1 3, the
number of design sprinklers is to be
the four most hydraulically demanding
sprinklers. The minimum required dis-
charge from each of the four sprinklers
is to be the greater of the following:
o The flow rates given in Table A for
NFPA 13D and 13R as a function of
temperature rating and the maxi-
mum allowable coverage area.
o A minimum discharge of 0.1 gpmisq.

Obstruction To Water Distribution.
Locations of sorinklers are to be in
accordance with the obstruction rules
of NFPA 13 for residential sprinklers.
Operational Sensitivlty. The sprin-
klers are to be installed relative to the
ceiling mounting surface as shown in
Figure 3.
Sprinkler Spacing. The minimum
spacing between sprinklers is 8 feet
(2,4 m). The maximum spacing be-
tween sprinklers cannot exceed the
length of the coverage area (Ref. Ta-
ble A) being hydraulically calculated
(e.9., maximum 12 feet for a 12 ft. x
1 2 ft. coverage area, or 20 feet for a 20
ft. x 20 ft, coverage area).
Installation
The Series LFll (TY2596) musl be in-
stalled in accordance with the follow-
ing instructions:ltorEs

Operation
When exposed to heat from a fire, the
Cover Plate, which is normally sol-
dered to the Support Cup at three
points, falls away lo expose the Sprin-
kler Assembly. At this point the Deflec-
tor supported by the Arms drops downto its operated position. The fusible
link of the Sprinkler Assembly is com-
prised of two link halves that are sol-
dered together with a thin layer of sol-
der. When the rated temperature is
reached, the solder melts and the two
link halves separate allowing the sprin-
kler to aclivale and flow water.
Design
Criteria
The Series LFll (W2596) Residential
Concealed Pendent Sorinklers are UL
and C-UL Listed for installation rn ac-
cordance with lhe following criteria.

mum of 21 ft.lbs. (28,5 Nm) of torqueis to be used to install sorinklers.
Higher levels of torque may distott the
sprinkler inlet with consequent leak-
age or impairment of the sprinkler.
Do not attempt to compensate for in-
sufficient adjustment in an Escutcheon
Plate by under- or over-tightening the
Sprinkler. Readjust the position of the
sprinkler titting to suit.
Step l. The sprinkler must only be
inslalled in the pendent position and
with the centerline of the sprinkler per-
pendicular to the mounling surface.
Step 2. Remove the Protective Cap.
Step 3. With pipe thread sealant ap-plied to the pipe threads, and using
the W-Type 18 Wrench shown in Fig-
ure 2, install and tighten the Sprink-
ler/Support Cup Assembly into the fit-ting. The W-Type 1 I Wrench will
accept a 1/2 inch ralchet drive.

Step 4. Replace the Protective Cap by
pushing it upwards until it bottoms out
against the Support Cup. The Protec-
tive Cap helps prevent damage to the
Deflector and Arms during ceiling in-
stallation and/or during application of
the finish coating of the ceiling. lt may
also be used to locate the center of the
clearance hole by gently pushing theceiling material against the center
point of the Cap.
NOTE
As long as the protective Cap remains
in place, the system is considered to
be "Out Of SeNice".
Step 5. After the ceiling has been com-
pleted with the 2-112 inch (63 mm)
diameter clearance hole and in prepa-
ration for installing the Cover Plate As-
sembly, remove and discard the Pro-tective Cap, and verify that the
Deflector moves up and down freely.
lf the Sprinkler has been damaged and
the Defleclor does not move up and
down freely, replace the entire Sprin-
kler assembly. Do nol attempt to mod-
ify or repair a damaged sprinkler.
Step 6. Screw on the Cover Plate As-
sembly until its flange comes in con-
tact with the ceiling.
Do not continue to screw on the Cover
Plate Assembly such that it lifts a ceil-
ing panel out of its normal position.
lf the Cover Plate Assembly cannot be
engaged with the Mounling Cup or the
Cover Plate Assembly cannot be en-gaged sufficiently to contacl the ceil-
ing, the Sprinkler Fitting must be repo-
sitioned.
Care and
MainJenance
The Series LFll (TY2596) must be
maintained and serviced in accord-
ance with the following instructions:

Absence of an Escutcheon Plate may
delay the sprinkler operation in a fire
situation.
Before closing a fire prctection system
main control valve for maintenance
work on the fire protection system
which it controls, permission to shut
down the affected fire protection sys-
tem must be obtained from the proper
authotities and all personnel who may
be affected by this action must be no-
tified.
Sorinklers which are found to be leak-
ing or exhibiting visible signs of corro-
sion must be replaced.
Automatic sprinklers must never be
painted, plated, coated, or otherwise
altered after leaving the factory. Modi-
fied or over heated sprinklers musl be
replaced.
Gare musi be exercised to avoid dam-age - before, during, and after instal-
lation. Sprinklers damaged by drop-ping, striking, wrench twisuslippage,
or the like, must be replaced.
The owner is resoonsible for the in-
spection, testing, and maintenance of
their fire protection system and de-
vices in compliance with this docu-
ment, as well as with the applicable
standards of the National Fire Protec-
tion Association (e.g., NFPA 25), in
addition to the standads of any other
authorities having jurisdiction. The in-
stalling contractor or sprinkler manu-
facturer should be contacted relative
to any questions.
NOTE
The owner must assure that the soin-
klers are not used for hanging oi any
objects and that the sprinklers are only
cleaned by means of gently dusting
with a feather duster; otherwise, non-
operation in the event of a fire or inad-
vertent operation may result.
It is recommended that automalicsprinkler systems be inspected,
lested, and mainlained by a qualified
Inspection Service in accordance withlocal requirements and/or national
cooes.
Limited
Warranty
Products manufactured by Tyco Fire &Building Products (TFBP) are war-
ranted solely to the original Buyer for
ten (10) years against defects in mate-
rial and workmanship when paid for
and properly installed and maintained
under normal use and service. This
warranty will expire ten (10) years
from date of shipment by TFBP. No
warranty is given for products or com-
ponents manufaclured by companies
not affiliated by ownership with TFBP
or for products and components which
have been subject to misuse, improper
inslallation. corrosion. or which have
not been installed, maintained, modi-
fied or repaired in accordance with ap-
plicable Standards of the National Fire
Protection Association. and/or the
standards of any other Authorities
Having Jurisdiction. Malerials found
by TFBP to be defective shall be either
repaired or replaced, at TFBP's sole
option. TFBP neither assumes, nor
authorizes any person to assume for it,
any other obligation in connection with
the sale of producls or parts of prod-
oress or implied. including warranliesof merchantability and fitness for aparticular ouroose-

sprinkler system design errors or inac-
curat€ or incomplete information sup-plied by Buyer or Buyer's repre-
sentatives.
In no event shall TFBP be liable, in
contract, tort, strict liability or under

indirect, special or consequential dam-
ages, including but not limiled to labor
charges, regardless of whether TFBP
was informed aboul the possibility of
such damages, and in no event shall
TFBP's liability exceed an amount
equal 10 the sales price.

This limited warranty sets forth the ex-
clusive remedy for claims based on
failure of or defect in products, materi-
als or components. whether the claim
