Dear Middle Creek Village LLC,
A review of the Town of Vail Community Development files indicates that the permit listed above
was never paid for or picked up. This permit was approved on April 24, 2009 and the applicant,
Stryker Site Services/Chris Stryker, was notified. Permits must be issued within 180 days of
notification or they become expired per the 2003 International Building Code, section 105.3.1, as is
the case here.
❑ If this work was never completed, the plan review fee must still be paid as we completed that
work in response to the application being submitted. The plan review fee is $293.70.
While contractors typically submit permit applications, property owners are ultimately responsible for
the permits on their property per the 2003 International Building Code, section 105.1.
Please note that if you have not completed this work but intend to do so in the future a new Design
Review Board (DRB) application and building permit application will be required. Your DRB
application expired 10/15/2009. If you apply for a building permit on or after January 1St, 2010, we
will be using the 2009 International Codes and plans submitted must show compliance with these
codes. The documents, including plans, that were submitted with your original application on
January 27th, 2009, will be recycled.

Chris,
Your building permit for the T-Mobile site (1309-0008) is ready to be picked up pending contractor
selection. All I need is a phone call or email from you letting me know who the contractor is so I can add
them in the computer (assuming they're already registered with us).
New contractors can register very easily if that's the case.
The fees due are $1,165.55.

B09-0008 was approved on April 24, 2009 at which time you were notified. This application has no
contractor listed, therefor we could not make it ready-to-issue.
This permit application will expire on October 24, 2009 if it is not paid for in full and picked up before
then. In order to be picked up, we must be notified who the contractor is, confirm they are registered
with us, and add them to the permit.
If this application expires we will also send you an invoice requesting payment for the plan review we
have performed, which totals $293.70. If you decide to move forward with the project after we have
expired the application, we will require a brand new, complete application submittal. Please note we
anticipate to adopt the 2009 International Codes this January, and any applications accepted are always
subject to the codes in effect at the time of application.
If you believe you are still moving forward with this project, please submit to us in writing an extension
request indicating your estimated timeline for picking up the permit and starting construction. This
request must be received before October 24, 2009 to keep the permit from becoming expired, however
the request will be subject to review by the Building Official and this email does not guarantee an
automatic approval of such a request.

Dear Mark:
As requested, Paul J Ford and Company has completed a review of the requested cabinets at the above referenced
site. Our review was based on Dynatek's layout drawings dated 03/16/09 and the base building drawings dated
08/09/02, both of which were provided to us by Dynatek on 03/23/09.
It is our understanding that the cabinets will be located on the lowest level of the existing parking garage structure
referred to as Level. P3 in the original construction documents. This level is at reference elevation 8252'-6".
Based on the original construction documents, the floor construction at this elevation is a 4" thick, normal weight
concrete slab on grade reinforced with WWF6x6-W2.1 xW2.1 wire mesh. Given this construction and the
proposed loading, this floor slab is adequate to support the cabinets as shown on Dynatek drawing A-1 dated
03/16/09.
