The permit must show work progressing within a 180 day period (an approved inspection must be completed) or an extension must be applied for per the 2009 International Building Code, section 105.5. The last approved inspection received on this permit was on 08/14/2007 and we do not show any record of an extension request. While contractors typically submit permit applications, property owners are ultimately responsible for the permits on their property per the 2009 International Building Code, section 105.1. I encourage you to. call for a final inspection so we may close this permit out and ensure all life safety components have been completed. Please contact us within 30 days as of the date of this letter to schedule this inspection. If we do not hear from you within 30 days, we will expire this permit. That means any future work you are considering could not be applied for or approved until this expired permit is resolved. Further, it may affect any future property sales should the prospective buyers research our records in Community Development for this property before purchase. As you can see, it is desirable to finalize the permit now so your property records with the Town of Vail are free of this outstanding issue and to ensure life safety for the inhabitants. If yes was answered to any of the questions then a "Public Way" permit is required. You can pick up an application at either Community Development, located at 75 South Frontage Road or Public Works, located at 1309 Elkhorn Drive. ➢ Notice sign-offs for utility companies. ALL utilities must field verify (locate) respective utilities prior to signing application. Some utility companies require up to 48 hours notice to schedule a locate. ➢ A construction traffic control/staging plan must be prepared on a separate sheet of paper. An approved site plan may also be used. This plan will show locations of all traffic control devices (signs, cones, etc.) and the work zone, (area of construction, staging, etc.). This plan will expire on November 1' and will need to be resubmitted for consideration for approval through the winter. Be aware that your resubmission for winter may be denied depending on the location of construction. ➢ Sketch of work being performed must be submitted indicating dimensions (length, width and depth of work). Sampling: During the sampling process, suspect ACM was separated into three EPA categories. These categories are thermal system insulation (TSI), surfacing ACM, and miscellaneous ACM. TSI includes all materials used to prevent heat loss or gain or water condensation on mechanical systems. Examples of TSI are pipe coverings, boiler insulation, duct wrap, and mud packed fitting cement. Surfacing ACM includes all ACM that is sprayed, trowelled, or otherwise applied to a surface. These applications are most commonly used in fireproofing, decorative and acoustical applications. Miscellaneous materials include all ACM not listed in the thermal system insulation or surfacing categories, such as linoleum, vinyl asbestos flooring, and ceiling tile. Boulder Environmental Management, Inc. followed EPA recommended sampling guidelines for identification of asbestos in bulk matrices. A set of three (3), five (5), or seven (7) samples were collected for each material type and analyzed by Polarized Light Microscopy (PLM). Each sample set was systematically analyzed, Upon determination that a sample in a set contained asbestos, analysis of the remaining samples in the set was discontinued. If no asbestos was detected during the analyses, the suspect material was determined to be negative for asbestos content. Materials that are identified as "non detected" are specified as not containing asbestos. Conclusion: The materials sampled do not contain asbestos. Applicable state, federal, and local environmental regulations stipulate that all "friable" asbestos containing building materials, `materials that can be crumbled by hand pressure', must be abated before prior to disturbance by renovation or demolition activities.
