Tnis application is for all changes to approved plans prior to Certiricate of Occupancy. An Appllcatlon for Design Review cannot be
accepted tmtll all required information is received by the Cornmunlty Development Department Design review approval lapses unless
a !building permit is issued and constructlon commences wlthln one year of the approval.

I. Three (3) copies of all pertinent approved plans with illustrated, labeled dwnges
