I arrived on site at ] 0:45 AM. I met with you to review the project requirements and specifications. The steel fabricator and erector for this project is Trinity Welding, Inc. in Leadville, Colorado. Trinity Welding Inc. fabricated material on site prior to erection; therefore, Periodic inspection shall be as required as stated in Section 1704.2 in the IBC for non-approved fabricators. The welder qualification test reports and welding procedure specifications for welders on site were reviewed and observed to be acceptable with project specifications. Copies of certifications aze on file with you. All column & beam material fabricated on site were inspected for proper fillet weld size, length, and location as indicated on supplied project drawings. Both the upper connection where the 3" pipe was welded to the saddle and the lower base plate, the bottom base plate was affixed to the grouted base with 5/8" anchor bolts. All material fabricated prior to erection was inspected and observed to be acceptable with visual requirements stated in the AWS D1.1 — Structural Welding Code: Section 6 and project requirements. Peterman Asbestos Testing, Project Design, and Consulting INTRODUCTION: On March 21, 2008 an inspection/survey was conducted and 5 bulk samples were collected from the: The Residence 1388 Vail Valley Drive East Unit Vail, CO. 81657 The purpose of the inspection/survey was to locate and sample suspected Asbestos containing materials that might be present in the area of the Resident Unit that is planned for selective renovation. The inspection was made, and the samples were collected by John R. Peterman, an A.H.E.R.A. and State of Colorado Certified Asbestos Inspector. Great care was taken during the inspection and sampling to be as accurate as possible. It should be noted that minimal damage was done to the existing building structures during the inspection so there is no documentation for unseen conditions or stored items. All samples were analyzed by DCM Science Lab in Wheatridge, CO. This laboratory is deemed "Proficient" in the E.P.A.Quality Assurance (QA) program for the determination of asbestos in buik materials, and is accredited by the American Hygiene Association (AHA). SAMPLING PROTOCOL: A random sampling scheme was used to sample the suspect materials that were discovered. If during any future demoiition or renovation work, suspect material is discovered that hasn't been sampled and would be disturbed, work should be halted until the material has been tested. The ceilings are covered in sheetrock like the walls; in some areas a new texture has been applied over the original surface. The floors are covered with carpet and ceramic tiles.
